Objective:To evaluate the performance of 10-2 visual field(VF)combined with macular ganglion cell–inner plexiform layer(GCIPL)measurements for detection of early glaucoma.Methods:A total of 93 eyes from 20 healthy participants,18 ocular hypertension(OH),18 glaucoma suspects(GS)and 37 early glaucoma(EG)were enrolled in this study.At the same time,we also divided all participants except the normal into three groups according to the Brusini Glaucoma Staging System 2(GSS2)standard: Stage 0,Stage B and Stage 1-2.Therefore,this study we used two grouping methods.All participants underwent 10-2 VF and conventional 24-2 VF testing using a Humphrey field analyzer and Macular GCIPL and peripapillary RNFL thicknesses and ONH parameters were measured using Cirrus High-definition Optical Coherence Tomography(Cirrus-HD OCT).The prevalence of abnormal 24-2 versus 10-2 visual field results based on cluster criteria,Area under the curve(AUC)of the receiver operating characteristic of GCIPL,RNFL and ONH parameters,correlation between visual field parameters and GCIPL,RNFL and ONH parameters in each diagnostic group was assessed.The diagnostic performance measures were determined after combining 24-2 visual filed result with RNFL,ONH parameters versus combining 10-2 visual filed result with GCIPL parameters.Results:1?In eyes with early glaucoma,glaucoma suspects and ocular hypertension group,11/21(52.4%),10/17(58.8%),3/15(20.0%)classified as normal on the basis of cluster criteria on 24-2 tests were classified as abnormal on 10-2 visual fields.A similar result is obtained by grouping according to GSS2.2?The AUCs of macular GCIPL in suspected glaucoma group and early glaucoma group was higher than that of optic disc RNFL,ONH parameter.The GCIPL parameters with the best AUCs were the minimum in early glaucoma and suspected glaucoma group.there is no statistically significance in ocular hypertension group.Group by GSS2,S 0,S B and S 1-2 all have the best diagnostic ability with minimum GCIPL.3?The 10-2 VF parameters have certain correlation with the GCIPL parameters in the early glaucoma group,suspected glaucoma and ocular hypertension group.Group by GSS2 only showed significant correlation between GCPL parameters and 10-2 visual field parameters in S1-2.4?The prevalence of abnormal combination of 10-2 visual filed result with GCIPL parameters(19 eyes)was significantly higher than 24-2 visual filed result with RNFL or ONH parameters(8 eyes)in 93 eyes we had collected(P < 0.05).Conclusions:Central visual field damage on the 10-2 test is often missed with the 24-2 strategy.Macular GCIPL measurements showed a high diagnostic capability to discriminate early glaucoma.Combinations of 10-2 visual filed result and GCIPL parameters is a great approach for the diagnosis of early glaucoma.This finding has implications for the diagnosis of glaucoma and classification of severity.
