Meta Analysis Of Risk Factors For Anastomotic Fistula After Colorectal Surgery

Abstract/Summary:
Objective:Anastomotic leakage was a serious complication of colorectal surgery,and there was no consensus on the risk factors of anastomotic leakage,and this meta-objective was to evaluate the risk factors of anastomotic leakage after colorectal resection.Methods:Through the computer retrieval PubMed Medline and Embase database,Google scholar English collection between January 2013 and January 2018 on postoperative anastomotic leakage related risk factors of colorectal retrospective and prospective studies using RevMan5.3 Meta analysis software,Newcastle Ottawa scale(NOS)bias evaluation,GRADE method for quality evaluation results of the level of evidence.Results:A total of 2882 literatures were retrieved,and 12 of them were eligible for inclusion after quality assessment.A total of 64,475 patients,including 3,152 patients with anastomotic leakage,with an incidence of 4.9%.This meta-analysis found that male OR=1.62 [95%CI :1.37,1.92] 、ASA≥3 OR= 1.45 [95%CI :1.34,1.57] diabetes OR=1.23 [95%CI :1.09,1.39] 、emergency surgery OR= 1.52 [95%CI: 1.52,1.92]、Smoking OR=1.71[95%CI:1.53,1.92] without intestinal preparation OR=1.46[95%CI:1.24,1.71] 、 preoperative radiotherapy and/OR chemotherapy OR=1.70[95%CI:1.39,2.08]may be risk factors for anastomotic leakage.Conclusion:Our analysis suggested that male,ASA ≥3,diabetes,emergency surgery,smoking,absence of intestinal para-tract,preoperative radiotherapy and/or chemotherapy were risk factors for anastomotic leakage after colorectal surgery.Recognition of these risk factors may influence treatment and process-related decisions and may reduce the incidence of anastomotic leakage.
