Objective :To explore the effect of CT-guided multi-channel drainage in the treatment of severe acute pancreatitis with Walled-off Necrosis.Methods :A retrospective analysis of 76 patients with severe acute pancreatitis with necrosis treated from March 2006 to October 2017.Divided into study group(n=48)and control group(n=28)according to different surgical methods.Among them,the patients in the study group underwent CT-guided multi-channel drainage of the abdominal cavity.The patients in the control group underwent retroperitoneal retroperitoneal approach to remove pancreatic necrotic tissue and indwelling the drainage of the thick three-lumen tube.Serum alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TBIL),glutamyl transpeptidase(GGT),interleukin-6(IL-6),interleukin-8(IL-8),and tumor necrosis factor alpha(TNF-?)were compared before and 7 days after surgery.The recovery time of blood amylase,the time of disappearance of abdominal pain,the time of disappearance of fever,postoperative hospital stay and postoperative complications were observed and recorded.Results : After treatment,the blood amylase recovery time,abdominal pain disappearance time,fever disappearance time and postoperative hospital stay time were significantly shorter in the study group than in the control group(P<0.05);the liverfunction indexes of serum AST,ALT,GGT and TBIL in the two groups were significantly lower than those before treatment(P<0.05),but the liver function indexes such as AST,ALT,GGT and TBIL in the study group were significantly lower than those in the control group(P<0.05);the inflammatory factors of serum IL-6,IL-8,TNF-?were significantly lower than those before treatment(P<0.05),but the level of inflammatory factors such as IL-6,IL-8,TNF-? in the study group were significantly lower than those in the control group(P<0.05);the total incidence of postoperative complications in the study group was 2.08%(1/48)significantly lower than that in the control group [14.29%(4/28),P<0.05].Conclusion : CT-guided multi-channel drainage in the treatment of severe acute pancreatitis wrapped necrosis is effective,can significantly improve the patient's inflammatory response and liver function,significantly reducing the incidence of postoperative complications.
