Clinical And Mechanism Study Of Buyang Huanwu Decoction Combined With Aspirin In The Treatment Of Chronic Portal Vein Thrombosis After Splenectomy

Purpose:This study mainly observed the clinical efficacy of Chinese herbal medicine "Bu Yang Huan Wu Tang"combined with aspirin in the treatment of chronic portal vein thrombosis after splenectomy and its clinical safety and possible mechanism.Method:In the first part of the clinical study,20 patients were randomly divided into treatment group and control group,10 cases in each group.The control group was given aspirin enteric-coated tablets 100 mg once a day,while the treatment group was given oral aspirin enteric-coated tablets 100 mg tablets.In addition,Fuyang Fluanwu Decoction was taken orally,morning and evening.The two groups were given basic rehydration and liver protection for a total of 1 month.The blood routine,coagulation function and liver function of the two groups were observed before and after treatment.Liver related indicators.'The second part of the basic research,mainly to observe the growth inhibition rate and cytotoxicity of the extracts on HUVEC cells,as well as the effects of TF,TFPI,tPA,PA I-1,ET-1 and other coagulation parameters mRNA.Result:Clinical studies showed that after treatment,platelet count(PLT),portal venous flow velocity(BFV),and portal vein diameter(PVD)were significantly lower in the two groups(P<0.05).There was no significant difference in prothrombin time(PT),international normalized ratio(INR)and activated partial thromboplastin time(APTT)between the coagulation function,and there was no statistical significance(P>0.05).Compared with the control group,the serum PLT and DDT were significantly decreased in the treatment group,while the ATIII was significantly increased,the difference was statistically significant(P<0.05).However,the treatment group had no significant difference in PT,INR,APTT,ALT and AST.There was no statistically significant change(P>0.05).From the Yerdel classification,the treatment group was significantly different from the control group,which was statistically significant(P<0.05).In the cell experiment section,it was found that Buyang Huanwu Decoction has certain protective effect on HUVEC cells and no cytotoxicity.On the other hand,this side can increase the expression of TFPI,inhibit the expression of TF,thereby regulating the activity of TF;it can increase the expression of t-PA mRNA,but has no significant effect on the expression of PAI-1 mRNA.Conclusion:This study found that Chinese medicine Buyang Huanwu Decoction combined with aspirin can significantly improve portal hemodynamics and correct the disorder of blood coagulation in the body,improve platelet aggregation,promote local thrombus dissolution,and have very good portal vein thrombosis after splenectomy.Good treatment.This experimental study provides a new clinical treatment and method for the treatment of portal vein thrombosis after splenectomy,but its specific mechanism needs to be further explored.Buyang Huanwu Decoction combined with aspirin for the treatment of chronic portal vein thrombosis after splenectomy is an effective and safe treatment plan,which provides a new choice for clinical treatment of this disease in the future.
