Objective:To study the anatomical variation of STA in hemorrhagic MMD patients and compare it with non-MMD patients.Materials and methods:From January 2017 to January 2018,50 cases(100 sides)of patients admitted to the department of neurosurgery of the first hospital of jilin university who were diagnosed as hemorrhage by CT and underwent head CTA examination were selected according to certain inclusion and exclusion criteria,including 25 cases(50 sides)in the MMD group and 25 cases(50 sides)in the non-MMD group.The selected cases were reconstructed at the image workstation to study the type of STA crossing the zygomatic arch,the relationship between the bifurcation and the zygomatic arch,the type of bifurcation and the location of bifurcation,and to measure the diameter and length of STA crossing the zygomatic arch.SPSS 25.0 was used to analyze the anatomical characteristics of STA,and P < 0.05 was considered to have significant difference.Result:1.The average age of patients in MMD group was 54.6±9.3 years old,the ratio of male to female was 1.1:1;in non-MMD group,the average age was 55.8±11.3 years old,and the ratio of male to female was 2.1:1.2.In the comparison of STA branch types,the difference between the MMD group and the non-MMD group was statistically significant(P<0.05),that is,the branch types of two groups were different,and type E(additional parietal branch)was relatively rare in MMD group(P<0.05).3.In the location of STA bifurcation,a(the distance from the zygomatic arch to the posterior margin of the mandibular condyle)had statistical significance between the two groups(P<0.05),that is,the STA bifurcation in MMD group was closer to the anterior margin of the auricle;b(the vertical distance between the bifurcation and the superior margin of the zygomatic arch)and c(the distance between the bifurcation and zygomatic process of frontal bone)had no statistical significance in the two groups.4.There was no significant difference between the two groups in the classification of the relationship between STA bifurcation and zygomatic arch position.There was no significant difference in the type of STA crossing the zygomatic arch between the two groups.There was no significant difference in diameter between the two groups at the beginning of STA,the midpoint of zygomatic arch and the bifurcation.There was no significant difference between the two groups in the length from the beginning of STA to the bifurcation.Conclusion:In this study,there were no significant anatomical differences between MMD and non-MMD patients in the length,diameter,type of cross-zygomatic arch and the relationship between bifurcation and zygomatic arch.According to the results of this study,there is no evidence indicated that MMD involves STA.In the MMD group,the STA bifurcation was closer to the front of the auricle and the branch type E was less common.These differences may be related to the congenital developmental abnormalities of MMD.This study lists the anatomical characteristics of STA in patients with MMD,which may provide a meaningful reference for vascular reconstruction of MMD.
