Objective: To use Computed tomography perfusion(CTP),Computed tomography angiography(CTA),Mini-Mental State Examination(MMSE),Montreal Cognitive Assessment(Mo CA)To evaluate the effect of surgical superficial temporal artery(STA)-middle cerebral artery(MCA)bypass on short-term cerebral hemodynamics and cognitive function in adult patients with moyamoya disease.Methods:Thirty inpatients diagnosed with moyamoya disease by DS A were selected as observation objects.All patients underwent unilateralSTA-MCA bypass surgery,and DSA,CTP,and CTA imaging examina tions were performed before and after one week of the operation;the perfusion parameters of the middle cerebral artery(MCA)distribution ar ea on the surgical side and the contralateral mirror control area were m easured,Including cerebral blood flow(CBF),cerebral blood volume(C BV),time to peak(TTP)and mean transit time(MTT),the relative C TP values(r CBV,r CBF,r TTP,r MTT)were obtained.Two neurocogniti ve tests of Mo CA and MMSE were performed one week before and 3 months after surgery.Results:1.30 cases of adult moyamoya disease patients showed different degrees of improvement in cerebral blood flow after surgery.The r CBF and r CBV of the middle cerebral artery distribution area and the contralateral mirror area after bypass surgery were higher than before surgery,and r MTT and r TTP were lower than those before surgery,P <0.05.Postoperative CTA showed that the anastomosis of the bridge vessel was good,the branch vessels were smooth and filling,and the wall was smooth.2.Mo CA and MMSE scores of 30 patients with moyamoya disease in the superficial temporal artery-middle cerebral artery bypass graft were improved compared with those before surgery,P<0.05.Conclusions:1.Surgical superficial temporal artery(STA)-middle cerebral artery(MCA)bypass surgery can improve short-term cerebral he-modynamics i n adults with moyamoya disease;2.CTP combined with CTA can effectively guide the surgical methods of adult moyamoya disease,evaluate the changes of postoperative cerebral perfusion status and the patency of bridge vessels;3.Surgical superficial temporal artery(STA)-middle cerebral artery(MCA)bypass surgery can improve short-term cognitive function in adults with moyamoya disease.
