The Influence Of Dexmedetomidine On Hemodynamics In Patient With Aldosteronism Undergoing Adrenal Tumor Excision

Objective:To evaluate the Influence of Dexmedetomidine on hemodynamics in patient with Aldosteronism undergoing adrenal tumor excision.Methods:Thirty patients undergoing adrenectomy were double blinded and randomly divided into 2 groups:Dexmedetomidine group(Dex group n=15)and control group(C group n=15).In Dexgroup,1.0ug/kg-h of Dex was administered intravenously over 10 minute,In C group,same volume of 0.9%normal saline was given.The SBP?DBP?MAP?HR were monitored.The parameters above were recorded at the time of entering operation room(baseline)(T1)?endotracheal intubation(T2)?the surgery begining(T3)?beginning to block tumor blood vessels(T4)?after tumor resection(T5)?end of the surgery(T6)?extubation(T7).Result:Demographic datas of these 2 groups were compared,and the difference had no statistical significance(P>0·05).The SBP?MAP?HR in Dexmedetomidine group were significantly lower than those of control group at the T2?T3?T4?T5?T6?T7 moment(P<0.05).The DBP in Dexmedetomidine group were significantly lower than those of control group at the T2?T3?T4?T5?T7 moment(P<0.05),but they were all within normal limits.Intra-group comparisons:HR?SBP of Dexmedetomidine group at T2?T3?T4?T5?T6 moment were significantly lower than baseline(T1)(P<0.05).DBP?MAP of Dexmedetomidine group at T3?T4?T5?T6 moment were significantly lower than baseline(T1)(P<0.05).The SBP,DBP,MAP of control group were higher than baseline at T2?T3?T4?T7 moment,while T5?T6 moment were lower than baseline,and the difference had no statistical significance;The HR of control group was higher than baseline(T1)at T2?T3?T7 moment,while T5?T6 moment is lower than recoredbaseline.the times intravenous injection of atropine was more than control group.Conclusion:Dexmedetomidine injected at a loading dose of lug/kg,followed by 0.2ug/kg·h pump injection made the hemodynamics more stable during adrenal tumor resection in patients with aldosteronism,but the change of heart rates should be paid attention to.
