Heart failure is a common cardiovascular disease.It is a complex clinical syndrome that makes the function or structure of the heart changed by a variety of causes,resulting in decreased heart filling ability and ejection function.It is also a global problem,and its high morbidity and mortality is a serious threat to human health.The occurrence of heart failure is accompanied by a variety of mechanisms,including oxdative stress,neurohumoral regulation mechanisms and the involvement of some cytokines.Therefore,in addition to some common clinical symptoms,heart failure often appears other problems,such as sodium water retention,electrolyte disorder and abnormal renal function.When heart failure occurs in the elderly people,the clinical manifestations are not typical due to their own physiological functions.So,when the elderly have the symptoms of heart failure or are treated in the hospital,the disease is often in a relatively serious stage,the prognosis is poor,the rehospitalization rate and mortality are high.Therefore,for the elderly,early diagnosis of heart failure is necessary.We mainly rely on the patient's symptoms,signs,blood samples,color Doppler echocardiography as a basis for the diagnosis of heart failure in clinical.The important roles of BNP(B-type natriuretic peptide)and NT-proBNP(N-terminal pro-BNP)are fully recognized in the 2017ACC/AHA/HFSA Heart Failure Management Guideline,it can not be carried out universally in some community or grass-roots hospitals,and color Doppler echocardiography is limited by some subjective factors or the technical level of technicians.Therefore,it is very important to find convenient,fast and universally applicable indicators to help doctors understand the diagnosis and prognosis of heart failure diseases.In clinical work,patients with heart failure are often accompanied by electrolyte disorder.A large number of clinical studies have shown that electrolyte disorder is closely related to the occurrence of heart failure.Objective:The Objective of this study is to establish a standard for predicting heart failure by analyzing the relationship between serum ions and heart failure,thus providing a theoretical basis for the diagnosis and treatment of elderly patients with heart failure.Methods:A total of 169 patients with heart failure in Geriatrics Department of the first hospital of Jilin University from May 2017 to June 2018 were selected as heart failure group.The heart failure group was divided into heart failure with reduced ejection fraction(EF<40%),heart failure with intermediate ejection fraction(40%-49%),and heart failure with preserved ejection Fraction(EF?50%)These three groups are marked as A,B,and C in turn.50 patients with non-heart failure who were hospitalized in the same time were selected as non-heart failure group.The age,sex,ejection fraction,triglyceride,cholesterol,low-density lipoprotein,high-density lipoprotein,serum otassium,sodium,chloride,and NT-proBNP levels were collected for all the patients.This research purpose is to establish a standard for predicting heart failure by analyzing the relationship between serum ions and heart failure,thus providing a theoretical basis for the diagnosis and treatment of elderly patients with heart failure.Results:1.There were statistical differences between NT-proBNP,LVEF,Na~+,Cl~-in heart failure group and non-heart failure group(P<0.05).2.EF has the greatest impact on heart failure,NT-proBNP,Na~+also have a certain impact.3.For the elderly,when sodium ion<139mmol/L,the possibility of heart failure is greater.4.NT-proBNP and EF have a strong prediction effect on heart failure,Na+,Cl-,have a certain prediction effect on different types of heart failure.5.IIn different types of heart failure patients,only NT-proBNP has a statistical difference(0.05)?NT-proBNP and K~+,which has different effects on different types of heart failure,and the NT-proBNP has a strong prediction effect on different types of heart failure,chloride ion also has a certain prediction effect.Conclusions:1.EF,NT-proBNP,Na~+have different degrees of influence on the occurrence of heart failure,EF has the greatest impact on heart failure.2.For the elderly,when sodium ion<139mmol/L,the probability of heart failure is higher.3.NT-proBNP,and K+had different effects on different types of heart failure.NT-proBNP has the largest value in the prediction of different types of heart failure,and chloride ion also plays a certain role in the prediction.
