Background and ObjectiveRectal cancer is one of the diseases with high morbidity,treatments have need to be enhanced and different prognosis for the similar treatment,and because the rectum belongs to the peritoneal external organs,the cancer metastasize is a particular way.Radical surgery can cause not only defecation incontinence,but also complications of sexual dysfunction speciall in men.Therefore,surgical treatment still has certain risks.In addition,the etiology of this disease is complex,and there is no consensus in academia.And generally speaking its are inchoate without distinctive sign and symptom,Usually partial patient cancer stage is terminal and the change that appears to be like habit of obstruction defecate just gives treatment.Currently,a large number of clinical studies have shown that misdiagnosis of early rectal cancer and patients' delay due to lack of attention are the main reasons for the increased morbidity and mortality of rectal cancer.Therefore,previous clinical studies have shown that commonly used gastrointestinal tumor markers(CEA CA19-9 CA72-4,etc.)can be used for early prediction of rectal cancer and postoperative and postchemotherapy detection,while recent foreign scholars have proposed that MPV(Mean platelet volume)in peripheral blood can be used for prediction and auxiliary diagnosis of early rectal cancer.However,neither the mature gastrointestinal tumor markers nor the newly proposed MPV indicators are very satisfactory for the screening and differential diagnosis of rectal cancer,especially for early rectal cancer.This experiment based on the above research background to investigate the diagnostic value of mean platelet volume(MPV)combined with CEA CA19-9 CA72-4 in early rectal cancer.Material and MethodRectal cancer patients admitted to the general surgery department of the second affiliated hospital of zhengzhou university from January 2014 to January 2019 were analyzed.Eligible cases were included: 50 cases in the normal group and 50 cases in the rectal cancer group.Clinical data of 50 patients in the normal group and 50 patients in the colorectal cancer group were collected,including basic clinical characteristic parameters,postoperative pathological data,preoperative blood routine,tumor markers and other data.Postoperative pathological data included TNM staging of the tumor.The fasting venous blood was extracted from the patients on the first day after admission,and examinations of blood routine and gastrointestinal tumor markers were conducted at the same time.The same treatment was performed for the normal group.In this study,all patients underwent transabdominal rectal cancer resection(low rectal anterior resection).Statistical methods: SPSS20.0 statistical software was used for data statistics,and the measurement data were expressed by mean standard deviation(x±s).Comparison between groups was conducted by group T test,and the counting data was analyzed by chi-square test and four-square table.The diagnostic value of mean platelet volume(sensitivity and specificity,etc.)for rectal cancer,P<0.05 was statistically significant.Results(1)MPV >13fL and CEA ?CA19-9? CA72-4 were significantly different between the normal group and the rectal cancer group(P<0.05).(2)The combined diagnosis method(combined MPV diagnosis)and non-combined diagnosis method(non-combined MPV diagnosis)were compared,and the results showed that the specificity of combined MPV diagnosis method was 68%.Sensitivity :86%,In contrast,the specificity of non-combined MPV diagnosis method was 78%,the sensitivity was 60%,Moreover,paied chi-square test(McNemar test)was performed on the combined MPV diagnosis method and the non-combined MPV diagnosis method,indicating that there was a statistical difference between the two methods(P<0.05).Conclusions(1)Comparison between the control group and the cancer group: there were no statistically significant differences in white blood cell,monocyte ratio,platelet count and age between the two groups(P > 0.05);There were statistically significant differences in MPV,neutrophil and lymphocyte ratios and hemoglobin(P < 0.05)(2)MPV is associated with early colorectal cancer and can be used to assist in the diagnosis of early colorectal cancer,but its diagnostic efficacy is similar to that of conventional tumor markers(by single item)(3)Combined MPV diagnosis is more sensitive than non-combined MPV diagnosis,and may be more suitable for the diagnosis of early rectal cancer...
