ObjectiveAnalysis of preoperative serum tumor markers in patients with rectal cancer, toassess the value of serum tumor markers in diagnosis and treatment of rectal cancer.MethodsThrough the review of Second Hospital Affiliated to Chongqing Medical Universityin2007March to2008March and underwent operation, postoperative pathologicalexamination confirmed the detection results of131patients with colorectal cancerpreoperative serum tumor markers. Calculate single or multiple serum tumor markers insensitivity of rectal cancer, analysis of relationship between serum tumor markers andclinical pathological parameters, and the relationship between serum tumor markers andsurvival. Normal reference range of serum tumor markers: CEA0-5.0ng/ml,CA199:0-37.0U/ml,CA724:0-6.7U/ml,CA242:0-20.0U/ml,CA125:0-35.0U/ml.Using SPSS19.0statistical software to analyze the data processing, count data usingthe chi-square test, survival curves estimated using the method of Klaplan-Meier, thecomparisons of survival curve between group using Log-rank test, analysis of factorsaffecting survival using Cox regression.ResultsIn the detection of serum tumor markers in a single, the sensitivity of CA724up to42.7%,and CA724>CEA>CA242>CA199>CA125;In the combined of two serum tumormarkers, CEA+CA724was the highest,64.1%;In the combined of three serum tumormarkers,CEA+CA724+CA125was the highest,66.4%; In the combined of four serum tumor markers,CEA+CA199+CA724+CA125was the highest,67.2%; In the combined ofall serum tumor markers, the sensitivity was66.4%; the difference of positive andnegative preoperative serum tumor markers in lymph node involvement vascularinvasion pTNM staging was statistically significant, positive are more likely to havelymph node invasion, lymphovascular invasion, pTNM staging later; Preoperativeserum tumor markers in CA199positive and negative there is a statistically significantdifference in recurrence and metastasis, positive were more likely to have recurrenceand metastasis; In patients with postoperative pathologic stage II in the patients, preoperative serum tumor marker in patients with CEA positive and negative in the5year survival rate of the statistically significant differences, positive patients havelower survival rates; In patients with postoperative pathologic stage III in the patients, preoperative serum tumor marker in patients with CA199CA242positive and negativein the5year survival rate of the statistically significant differences, positive patientshave lower survival rates; Multivariate COX regression analysis of CEA CA199CA125,they can be used as factors influencing the survival conditions.ConclusionCombined detection of serum tumor markers can significantly improve thesensitivity, but there is no great difference sensitivity of two, three, four, five jointdetection; CEA for tumor stage, lymph node metastasis, vascular invasion, survival rateand as the factors influencing the survival is of certain value; CA199for tumor stage, lymph node metastasis, vascular invasion, recurrence or metastasis, survival rate and asthe factors influencing the survival is of certain value;CA125has a certain value as asurvival factor;CA242has a certain value for tumor survival rate of patients;CA724don’thave the relationship with all of the above factors.
