Objective1.To investigate the physical activity of peritoneal dialysis patients and analyze its influencing factors.2.Based on the investigation of influencing factors in the first stage,an aerobic exercise intervention program for peritoneal dialysis patients with low physical activity was constructed,and its application effect was discussed.MethodsThis study is divided into two stages: questionnaire survey and intervention study.1.Inventory surveyFrom December 2017 to May 2018,patients over 3 months and 18 years old who received peritoneal dialysis treatment in two third-level first-class hospitals in Zhengzhou City were selected as the subjects of study.Questionnaires were conducted to investigate the general information,physical activity,social support,exercise self-efficacy and hope level of peritoneal dialysis patients.Factors affecting physical activity,analyze the data accordingly by IBM SPSS Statistics 21.0 statistical software.Descriptive statistics,one-way ANOVA,t-test and multiple linear regression were used to analyze the collected data.The test level was ?=0.05.2.Intervention studyAccording to the first part of the experimental study,the intervention program of aerobic exercise was constructed.From June 2018 to February 2019,90 patients in peritoneal dialysis center of a third-class A hospital in Zhengzhou were selected as the study subjects.The patients were divided into two groups on average,the control group and the intervention group with 45 cases each by coin flipping method.Patients in the control group received routine education,while those in the intervention group received aerobic exercise intervention program on the basis of routine education.The intervening time was 3 months.The physical activity,six-minute walking distance,STS10 standing seconds,exercise self-efficacy level,positive mental health and quality of life data,serum creatinine,urea nitrogen and other clinical indicators were collected before,after and 3 months after intervention.The collected data were analyzed by descriptive statistics,t-test,rank sum test,chi-square test,generalized estimation equation and repeated measurement variance analysis.Results1.Inventory surveyA total of 248 questionnaires were completed in this study,230 of which were valid,with an effective rate of 92.7%.(1)The metabolic equivalent value of physical activity in home-based peritoneal dialysis patients was(929.52 +1326.09)MET.Among them,164(71.3%)were low physical activity,34(14.8)were moderate physical activity and 32(13.9)were high physical activity.Exercise self-efficacy score was(39.85 + 18.30),social support score was(36.36 + 8.26),hope level score was(30.10 + 8.60).(2)The influencing factors of physical activity of home-based peritoneal dialysis patients were analyzed by single factor analysis of variance,chi-square test and t-test.The results showed that gender,education level,working status,exercise status,number of complications,hemoglobin and serum albumin were the important influencing factors of physical activity of home-based peritoneal dialysis patients.(3)Regression results showed that exercise status,number of complications,hemoglobin and serum albumin,exercise self-efficacy,social support and hope level were the important influencing factors of physical activity.2.Intervention studyIn the course of intervention,5 cases of home-based peritoneal dialysis patients were lost,85 cases completed the intervention completely,43 cases in the intervention group and 42 cases in the control group.(1)The results of metabolic equivalent of physical activity,six-minute walking distance and STS 10 sitting-up seconds before and after intervention showed that there were significant differences in inter-group effect,time effect and interaction effect(P < 0.05).(2)The scores of exercise self-efficacy,positive mental health and quality of life of peritoneal dialysis patients before and after intervention were significantly improved compared with those before intervention and the control group.(3)The effect of intervention on hemoglobin,serum albumin and peritoneal dialysis adequacy(serum creatinine,urea nitrogen,KTV value)remains to be studied.Conclusions1.Physical activity of peritoneal dialysis patients in this study was mainly affected by hemoglobin,exercise status,number of complications,exercise self-efficacy,social support and hope level.2.The aerobic exercise intervention program in this study can improve the physical activity level of peritoneal dialysis patients,enhance exercise self-efficacy,promote positive mental health and improve the quality of life of patients.Although the improvement of hemoglobin,albumin and adequacy of peritoneal dialysis is not obvious,it still has a certain improvement effect.
