Clinical Observation On Treatment Of Intertrochanteric Fracture With PFN-A By TCM Differentiation

Objective:Through the observation and analysis of PFN-A of femoral intertrochanteric fracture after routine treatment with TCM syndrome differentiation and treatment of stage of femoral intertrochanteric fracture with PFN-method after A treatment compared with conventional fracture,provide new choice and reference to promote the rehabilitation of bone healing of femoral intertrochanteric fracture after operation.It embodies the complementary advantages of the combination of Chinese and Western medicine.Methods : Methods from September 2016 to December 2017,80 patients with intertrochanteric fractures who met the inclusion criteria were selected in the Department of bone surgery of Shanxi Hospital of integrated traditional Chinese and Western medicine.Were randomly divided into two groups(40 patients in each treatment group and control group),were treated by PFN-A procedure,the treatment group: the three dialectical Chinese medicine treatment of fractures: early: 1-2 weeks after fracture,the blood stasis,detumescence and acesodyne;Xuefuzhuyu Decoction;fracture of middle fracture: 3-4weeks after the renewal tendon bone;Xuguhuoxuetang fracture: late addition and subtraction;5-6 weeks after fracture,with Replenishing Qi and blood,nourishing liver and kidney,raising tendons and bones.Zhuang Jin and blood nourishing soup is added and reduced.The first day after the operation started taking prescription,continue to take 30days(three prescriptions each for 10 days),the rest of the routine treatment.The control group was treated with routine treatment,postoperative rehabilitation measures are the same,to observe and analyze and record the postoperative hospitalization time,hospitalization expenses,drug costs,complications,the number of types,manifestations,complications,postoperative 3 day,1 week,2 weeks and 3 weeks,VAS score,postoperative 8 weeks,12 weeks the rate of fracture healing,hip Harris 3 months and 6months postoperative score.The two groups were treated with the standard of fracture clinical healing.Results :Compared with the control group,the VAS score of the treatment group was lower than the control group at third,7,14 and 21 days after the operation,indicating that the treatment group had a certain analgesic effect on improving the pain of the hip joint after PFNA.8 weeks after PFNA,the healing rate of fracture in treatment group andcontrol group in February was 90% and 75%,respectively,indicating that the healing effect of PFNA in the treatment group was better than that in the control group at 8 weeks after PFNA.12 weeks after PFNA,the fracture healing rates both in the treatment group and the control group were 100%.It could be considered that there was no significant difference in fracture healing rate between the treatment group and the control group at 12 weeks after operation.Similarly,there was no significant difference in Harris score between the two groups before and after PFNA operation(P>0.05).Conclusion:The patients in the treatment group and the control group recovered well after operation,but the treatment group had more stable effect,faster recovery and shorter fracture healing time after the treatment of Chinese medicine,and effectively prevented fracture complications.
