Objective: because of the narrow alveolar bone and the restriction of anatomic structures,implantation in the area of mandibular anterior teeth has been difficult in clinical practice.There are differences in the mandibular protrusion,lateral and cycle movement between the anterior crossbite and the normal subjects.Therefore,not only the implant position should be considered in the implant restoration of the anterior crossbite,but also the regularity of the occlusal movement.The main purpose of this observation is to evaluate the clinical effect of implant restoration of anterior teeth in patients with anterior crossbite,to explore the characteristics of the alveolar bone in the mandibular anterior area of the patients with crossbite and its influence on the implant,and to understand the implant characteristics of the anterior crossbite in depth.Data and methods: five patients with anterior crossbite and require for implant restoration were selected,each has single or multiple anterior teeth missed,or unable to maintain.The width of alveolar bone and the thickness of labial and lingual cortical bone in the implant area were analyzed by X-ray film before operation in order to preset the position of the implant.The average follow-up period was 12 months(from 9 to 15 months).The clinical efficacy was evaluated from several aspects:(1)the implant condition and width of alveolar bone(2)restoration and occlusal condition(3)patient's satisfaction evaluation.Results: there were 9 implants in all,immediate implant in 3 cases,delayed implant in 2 cases,single tooth missing in 1 case,multiple teeth missing in 4 cases.All the implants are OSSTEM MS 3.5*10mm.No implant showed loosening,shedding,breaking,etc.The soft tissue around the implant was healthy,showed no peri-implant inflammation.The mean width of alveolar crest was 5.60 mm in 5 patients.All the implant sites were thicker in the lingual cortex than in the labial side.The average thickness of the cortical bone in the labial side was 0.75 mm,and the lingual cortical bone was 1.45 mm.All the restorations were bonding retention,and the occlusal relationship of ultimate restoration was reversed.The patients were satisfied with the results of treatment.All the patients were followed up to investigate the patient's subjective satisfaction.The average scores of mastication,pronunciation,comfort and overall satisfaction were above 80,only the average scores of bite compared with their own teeth and beauty were about 73.Conclusion:(1)The average width of alveolar bone was 5.60 mm in the implant area in 5 patients,which was suitable for the implantation of narrow diameter implants.The lingual cortical bone in the crest of alveolar ridge was thicker than that in labial side.(2)The anterior crossbite can be corrected only if the vertical distance is increased with full occlusal reconstruction.No occlusal reconstruction of the posterior teeth was carried out in all patients,so the anterior repair was still in reverse state.(3)The anterior crossbite did not necessarily cause adverse effects on the masticatory system such as joints and muscles.Good clinical results can be obtained when the ultimate implant restoration of the mandibular anterior teeth is still crossbite...
