Objective: to investigate the clinical effect of plasma adenoid radiofrequency ablation under nasal endoscope in patients with chronic rhinosinusitis and its effect on patient outcomes.Methods: 120 cases of children with chronic rhinosinusitis with adenoid hypertrophy admitted to the department from July 2016 to December 2018 were selected and divided into control group(n=60 cases)and observation group(n=60 cases)by random number method.The control group was treated with nasal spray,corticosteroids and oral mucosal ciliary function restorer,while the observation group was treated with nasal endoscopic plasma adenoid radiofrequency ablation postoperativetreated with nasal spray,corticosteroids and oral mucosal ciliary function restorer.The patients in the two groups were evaluated for 30 days of continuous treatment,and the patients were followed up for 6 months after the course of treatment.The symptoms of nasal obstruction,runny nose,headache,hyposmia and nasal mucosa congestion and swelling were recorded and statistically analyzed.Visual analogue pain(VAS)scale was used to evaluate subjective symptoms at different time points in the two groups.Lund-Kennedy nasal endoscopic scale was used to evaluate the symptoms of sinusitis in the two groups.Recurrence rate and complication rate were recorded 6 months after treatment.Results :(1)the short-term efficacy rate of the observation group after radiofrequency ablation of plasma adenoid under nasal endoscope was 95.00%,higher than that of the control group(76.67%)(P<0.05).(2)there was no significant difference in the disappearance time of nasal obstruction,runny nose and headache between the two groups(P>0.05).The duration of disappearance of hyposmia and nasal mucosal congestion and swelling in the observation group was shorter than that in the control group(P<0.05).(3)there was no significant difference in VAS scores between the two groups before treatment,15 days after treatment and 30 days after treatment(P>0.05).VAS scores of the observation group and the control group were lower at 1d,5d,15 d and 30 d after treatment than before treatment(P<0.05).VAS scores of 5d and 15 d after operation in the observation group were lower than those in the control group(P<0.05).(4)the scores of polyps,edema,rhinorrhea,scar and scab in the two groups before treatment were not statistically significant(P>0.05).The scores of polyp,edema,rhinorrhea,scar and scab in the observation group were all lower than those in the control group 30 days after treatment(P<0.05).(5)patients were followed up for 6months after the completion of treatment,and no statistical significance was found in the recurrence rate comparison between the two groups at 1 month after treatment(P>0.05).The recurrence rate of the observation group was lower than that of the control group at 3 and 6 months after treatment(P<0.05).(6)there was no significant difference in the incidence of adverse reactions during the treatment between the two groups(P>0.05).Conclusion: radiofrequency ablation of plasma adenoids under nasal endoscopy can achieve higher short-term efficacy in patients with chronic rhinosinusitis with adenoid hypertrophy,shorten the time of symptom disappearance,In addition,the recurrence rate of patients after treatment is low,without increasing the incidence of complications,which is worthy of promotion and application.
