Objective:Through the study of the current status of caries and periodontal health in the 12-15-year-old population in Jiangxi Province,we can understand the oral health-related behaviors of 12-15 year-olds in Jiangxi Province.To grasp the incidence of dental caries and periodontal diseases in different characteristics of population,find out the main influencing factors of oral health in this age group.To provide scientific basis and Countermeasures for formulating appropriate oral health promotion strategies for key areas and key issues by Jiangxi health departments.Methods:A total of 8160 cases of 12-15 years old people were selected from 10 counties(districts)in 5 cities of Jiangxi Province by stratified cluster sampling method.Oral clinical examination was performed by three doctors with more than ten years' experience in oral clinical work.Diagnostic criteria and questionnaire filling criteria were carried out according to the fourth national oral epidemiological survey program.Epidata3.0 software is used to input data.The SPSS20.0 is used for data analysis.Statistical descriptions were made on the rate,and constituent ratio of counting data,Chi-square test,rank sum test,binary logistic regression was used for univariate analysis and multivariate analysis of caries and periodontal health.Results:(1)Current Situation of Knowledge,Attitude,and Behavior of 12-15-year-old Population in Jiangxi Province.(1)Knowledge awareness rate: The average awareness rate of 8 oral health knowledge points was 57.18%.Among them,fluoride could protect the teeth 43.81%;sugar could cause caries 65.85%;bacteria could cause caries 50.22%;brushing could prevent gingivitis 72.18%.(2)Correct attitude rate: regular oral examination 68.82%,The quality of teeth is innate 92.16%,prevention of dental disease by oneself 92.84%,oral health is very important 94.2%.(3)Oral related behaviors: the rate of brushing teeth twice or more per day was 26.23%,the rate of using fluoride toothpaste was 51.17%,and the rate of using dental floss was 0.44%.Frequency of dessert and candy consumption(rarely/never 13.80%,once a week 14.67%,more than 2 times a day11.65%),frequency of sugary drinks consumption(rarely/never 33.90%,once a week16.78%,> 2 times per day3.26%).(2)Epidemiological characteristics of caries among people aged 12-15 in Jiangxi Province: The caries rate of permanent teeth was 25.81%(95%CI :24.86%-26.76%),the DMFT was 0.48,and the sputum supplement ratio was 14.10%.The age groups of 12-15 years old were 24.36%,26.52%,26.52%,and 25.83% respectively.The caries rates in five areas were 29.41% in Nanchang,29.17% in Jiujiang,18.69% in Ganzhou,19.49% in Yichun and 32.29% in Shangrao.The caries rate in rural areas was 29.34% and DMFT was0.56 higher than that in urban areas 22.28% and 0.41.The incidence of caries in females was 28.90% higher than that in males 22.72%(P < 0.05).Multivariate logistic regression showed that: rural vs city OR = 1.489(95% CI: 1.345-1.649);female vs male OR=1.385(95% CI: 1.252-1.533);Jiujiang vs Nanchang OR=0.980,Ganzhou vs Nanchang OR=0.550,Yichun vs Nanchang OR =0.586,Shangrao vs Nanchang OR=1.156.The higher the frequency of sugar milk intake,the greater the risk of caries.(3)Epidemiological characteristics of gingival bleeding among people aged 12-15 in Jiangxi Province: The gingival bleeding rate of permanent teeth was 66.52%(95% CI: 65.50%-67.54%).The average number of bleeding teeth was 4.65.The age groups of 12-15 years old were 65.10%,65.83%,65.39%,and 69.75% respectively.The rates of gingival bleeding were 81.62% in Nanchang,62.01% in Jiujiang,58.09% in Ganzhou,62.68% in Yichun and 68.20% in Shangrao.The prevalence of gingival bleeding in urban areas was 68.04%,and the average number of bleeding teeth was 4.94 higher than 65.00% and 4.36 in rural areas(all P<0.05).The incidence of gingival bleeding in males was 67.62% higher than that in females 65.42%(P <0.05).Multivariate logistic regression showed that the older the age,the higher the risk of gingival bleeding;rural vs.city OR = 0.840;Jiujiang vs.Nanchang OR=0.361;Ganzhou vs.Nanchang OR=0.297;Yichun vs.Nanchang OR =0.363;Shangrao vs.Nanchang OR =0.455.The frequency of brushing teeth per day <1 vs.?2 per day.OR =1.590.(4)Epidemiological characteristics of dental calculus among people aged 12-15 years in Jiangxi Province.The calculus rate of permanent teeth was 70.54%(95% CI: 69.55%-71.53%).The average number of calculus teeth was 4.35.The age groups of 12-15 years old were 63.38%,69.17%,72.21%,and 77.40% respectively.The incidence of dental calculus was 58.95% in Nanchang,65.32% in Jiujiang,72.98% in Ganzhou,77.21% in Yichun and 78.25% in Shangrao,respectively.The prevalence in the urban area was 73.31%,and the average number of calculous teeth was 4.64,which were higher than that in rural area 67.77% and 4.05 respectively,(P<0.05).The prevalence of male was 72.35% higher than that of female 68.73%(P < 0.05).Multivariate logistic regression showed that the older the age,the higher the risk of dental calculus;rural vs.city OR = 0.689;female vs.male OR = 0.868;Jiujiang vs.Nanchang OR =1.285;Ganzhou vs.Nanchang OR = 1.780;Yichun vs.Nanchang OR = 2.366;Shangrao vs.Nanchang OR = 2.366.Mother's education level university and above vs.primary school and below OR=0.259;the frequency of brushing teeth per day <1 time vs.?2 times per day OR=1.330.Conclusion:The prevalence and severity of dental caries among 12-15 year-olds in Jiangxi Province were lower than the national average,but the supplementary filling of dental caries was poor.It shows the characteristics of female higher than male,rural higher than urban,northern higher than southern,Eastern higher than western.The prevalence of gingival bleeding and dental calculus was higher than the national average and showed the characteristics of male higher than female,urban higher than rural.The health department should develop an oral health plan that is inclined to the disadvantaged areas.And equitably allocate oral health resources.It should focus on the cultivation of oral health and hygiene behaviors such as eating habits and brushing habits of people aged 12-15.Based on community,school and family,oral health education should be carried out to improve oral health,reduce the burden of oral diseases and improve the quality of life.
