ObjectiveOsteonecrosis of the Femoral Head(ONFH)is a destructive joint disease caused by multiple factors and is one of the clinical problems that need to be solved urgently.Extracorporeal Shock Wave(ESW)is a sound wave with high pressure and high speed.A large number of studies have proven that ESWT has a great role in the treatment of ONFH.Core Decompression(CD)is the most commonly used procedure in the clinic.Both methods rely on accurate localization of necrotic areas within the femoral head.This study intends to reconstruct the three-dimensional model of the femoral head of patients with early ONFH patients by MRI three-dimensional reconstruction technology,and to model,mesh and assign values,and apply mechanical analysis software to mechanically analyze the femoral head,and to develop individualized,Accurate surgery and ESWT treatment plan,and develop individualized rehabilitation guidance after treatment,and compare with conventional surgery and ESWT treatment to evaluate the effect of individualized and precise treatment of ONFH.MethodsIn this study,52 patients(84 hips)with ONCO patients with stage I and II ARCO were selected.They were randomly divided into Experimental group and control group.General data were collected.The Experimental group and the control group were treated with decompression and decompression under epidural anesthesia after perfect preoperative examination.ESW treatment was started 1 week after operation.The therapeutic effects were evaluated by visual analogue scale(VAS),Harris score,and three-dimensional technique to measure the volume of necrotic area.All patients were followed up for at least 15 months.ResultsThe VAS scores of the Experimental group and the control group at 6 months,9 months,and 15 months after treatment were significantly improved compared with those before treatment,and the difference was statistically significant(P < 0.05).The VAS scores of the Experimental group and the control group were statistically significant(P < 0.05)before and after treatment,6 months,9 months,and 15 months after treatment.The Harris scores of the Experimental group and the control group at 6 months,9 months,and 15 months after treatment were significantly improved compared with those before treatment,and the difference was statistically significant(P < 0.05).The Harris scores of the Experimental group and the control group were statistically significant before treatment and at 6 months,9 months,and 15 months after treatment(P < 0.05).The size of the femoral head necrosis area was constructed according to the 3D reconstruction technique.The Experimental group was statistically significant 9 months after treatment(P < 0.05)and 15 months(P < 0.05).The control group was 9 months after treatment.(P < 0.05)and 15 months(P < 0.05)were statistically significant compared with before treatment.The necrotic volume was statistically significant(P<0.05)before treatment and at 9 and 15 months after treatment.At a minimum follow-up of 12 months,the clinical outcome of the Experimental group was 82.22%(37/45),13.33%(6/42),and 4.44%(2/42).The clinical outcome of the control group was 71.79%(28/39),15.38%(6/39),and 12.8%(5/39).ConclusionsBased on MRI-3D reconstruction technology to guide drilling decompression and ESWT treatment of ONFH,and through the mechanical analysis technology to guide individualized rehabilitation program,can effectively improve the efficiency of drilling decompression and ESWT,improve patient satisfaction.
