Objective To analyze the correlation between blood lipid-related index level and chronic hepatitis B virus infection,and to explore the clinical significance of blood lipid-related index level and Albumin-bilirubin index(ALBI index)on the disease progression after chronic hepatitis B virus(HBV)infection,and to further evaluate the clinical value of blood lipid,its related index level and ALBI index as indicators for predicting liver function damage degree after chronic hepatitis B virus(HBV)infection.Methods 184 patients with chronic HBV infection(74 patients with hepatitis B virus,70 patients with chronic hepatitis B and 40 patients with posthepatitic cirrhosis)and 50 healthy controls were selected as the research objects,and the basic data of each group were collected: age,sex,height and weight.Relevant indexes were detected: total TClesterol(TC),triglyceride(TG),high density lipoprotein TClesterol(HDL-C),low density lipoprotein TClesterol(LDL-C),apolipoprotein A(APOA),apolipoprotein B(APOB)and alanine aminotransferase(ALT),oryzae sativa aminotransferase(AST),albumin(ALB)and total bilirubin(TBIL).184 patients with chronic HBV infection and healthy controls were analyzed without grouping.T test was used to make statistics on the data and analyze the difference between healthy controls and patients with chronic HBV infection in blood lipid related index levels.According to the formula,ALBI index was calculated and recorded.184 patients with chronic HBV infection were divided into HBV carrier group,chronic hepatitis B patient group and hepatitis B related cirrhosis group according to the diagnostic criteria of the guidelines.Combined with healthy control group,the data were statistically analyzed by one-way variance analysis to study the change characteristics of blood lipid related index level and ALBI index in disease progression after chronic HBV infection.Without grouping,the correlation analysis of ALBI index and corresponding lipid-related index levels of all the participants was made to explore the value of lipid index and ALBI index in evaluating the degree of liver damage.Results Compared with the control group,serum lipids in patients with chronic HBV infection group were significantly reduced,with HDL-C and APOA having statistical significance(P< 0.05).With the progress of the disease,the concentration of blood lipid indexes in patients with chronic HBV infection gradually decreases.the serum TC(3.28±1.02 mmol /L),TG(0.99±0.43 mmol /L),HDL-C(0.77±0.34 mmol /L),LDL-C(2.10±0.64 mmol /L),APOA(1.16±0.53 mmol /L),APOB(0.57±0.19 mmol /L)levels in liver cirrhosis group are significantly lower than those in the control group(p<0.05).ALBI index gradually increased with disease progression(P<0.05).serum TC,TG,HDL,APOA,APOB levels were negatively correlated with ALBI index,among which TC,HDL-C and ALBI index had higher negative correlation coefficients,0.301,0.266 respectively,P<0.05.Conclusions Chronic HBV infection is associated with lower serum HDL-C and APOA levels.Elevated ALT is also an indicator of progression of chronic hepatitis B patients.The calculation of ALBI index has certain clinical significance in the process of evaluating the condition of non-end-stage chronic liver diseases.Blood lipid-related index concentration in patients with chronic HBV is helpful to reflect the degree of liver function damage,especially in patients with liver cirrhosis.
