Purpose:To evaluate the relationship between the maximal elasticity value(Emax)of quantitative parameters and the clinicopathological factors in the shear wave elastography(SWE)technique of breast cancer,and to preliminarily evaluate the pathological characteristics of breast cancer,so as to provide some auxiliary value for early diagnosis,prognosis evaluation and precise treatment of breast cancer.Methods:76 patients(76 lesions)with breast cancer confirmed by surgery in the breast surgery ward of Nanfang Hospital of Southern Medical University from January 2017 to July 2017 were included in the clinical study.Conventional ultrasound and shear wave elastography were performed on 76 breast cancer lesions prior to breast cancer surgery.The lesion size was measured in conventional ultrasound,and the parameter Emax was obtained from the shear wave elastography results.The following clinicopathological factors were collected in the postoperative pathological results:pathological type,axillary lymph node metastasis,immunohistochemical characteristics(estrogen receptor,progesterone receptor,Human epidemal growth factor receptor 2 and ki-67 levels)and molecular subtypes.Univariate and multiple linear regression methods were used to analyze the relationship between Emax and clinicopathological factors of breast cancer.Result:1.The difference in Emax between different lesion sizes was statistically significant.As the tumor increased,the Emax value also increased(p<0.001);2.There was a significant difference between Emax in the HER2 positive expression group and the negative expression group.The Emax of the HER2 positive expression group was greater than that of the negative expression group(p=0.01).3.The Emax of tumors in different molecular subtypes was significantly different(p=0.021),and the Emax of the four molecular subtypes from large to small was:Lminal B type>HER2 overexpression type>Luminal type A>three negative type.4.There was no significant difference in the expression of Emax between different pathological types,ER positive group and ER negative group,PR positive group and PR negative group,and different ki-67 expression levels.5.The statistically significant parameters(lesion size,lymph node metastasis,HER2 and molecular subtypes)in the above-mentioned univariate analysis were included in the multivariate linear regression analysis.The results showed that Ema had the largest relationship with lesion size and molecular subtype.Conclusions:1.The shear wave elastography quantitative parameter Emax has a certain relationship with breast lesion size,axillary lymph node metastasis,HER2 expression and molecular subtype.Among them,the relationship between Emax value and breast lesion size and molecular subtype is the largest.The hardness values between the four molecular subtypes are different,wherein the Lmax of the Lminal B type is the largest and the hardness value of the three negative type is the smallest.Emax can reflect the biological behavior of breast cancer mass to a certain extent.
