| ObjectivesTo investigate the prevalence and risk factors of anxiety, depression and perimenopause syndrome among middle-aged and elderly female of HIV/AIDS patients in Anhui Province.MethodsConsider north Anhui Province have older HIV/AIDS patients more than other areas in Anhui,it was selected as the research site.A cluster random sampling method was used to select 8 townships in 3 counties and districts, above 40-year-old female HIV/AIDS patient and its peers were recruited as study objects. The survey included sociodemographic characteristics, health status, birth information, anxiety, depression and perimenopause syndrome and related information such as the number of CD4 cells.EpiData 3.1 was used to record data, and database classification and statistical analysis were performed by R-3.5.1 software.Continuous variables are expressed in the form of,categorical variables are expressed as a percentage(%),Pearson Chi-square,Student’s t-test,and Mann-Whitney U test are used to compare differences between groups. The factors influencing anxiety, depression and perimenopause syndrome were analyzed by unconditional logistic regression using two-class dependent variables, and the test levelα=0.05.ResultsA total of 521 people was investigated in this study, finally analysis including 495people with 240 HIV/AIDS patients and 255 control populations. The average age of the HIV/AIDS patients group was(52.4±6.11)years,and the illiteracy rate was 65.8%.The population of HIV/AIDS patients was mainly from single-slurry blood transfusion population(87.1%),the average treatment time was(11.5±3.69)years,and the number of 44.1%CD4 cells exceeded 500/mm~3.The average age of the non-HIV-infected group was(53.3±7.23)years,and the illiteracy rate was 57.8%.Compared with non-HIV-infected people,HIV/AIDS patients had a higher proportion of non-partners(P<0.05),family income was less than 10,000 yuan in a higher proportion(P<0.05),sleep quality was worse(P<0.05),and poor visual acuity and more people with poor hearing(P<0.05).The average score of depression in HIV/AIDS patients was(8.15±3.81)points,and the detection rate of depressive symptoms was 61.4%.The average anxiety score was(51.4±9.84)points,and the detection rate of anxiety symptoms was 65.5%.The average score of perimenopause syndrome was(18±10.6)points,and the detection rate of perimenopause syndrome was 85%,which was all higher than control group, the difference was statistically significant(P<0.05).The anxiety, depression, and perimenopause syndrome overall score were related to each other.Univariate analysis showed that there were higher scores of depression in HIV/AIDS patients than those in the control group when HIV/AIDS patients and non-HIV-infected people scored the same in anxiety scores or perimenopause syndrome scores.Perimenopause syndrome as a mediating role between anxiety and depression.Perimenopause women with anxiety can influence depression indirectly through perimenopause syndrome.HIV infection is an independent risk factor for depressive symptomsThe results of two-category unconditional logistic regression analysis showed that the risk factors of anxiety were low family income, anxiety, perimenopause syndrome and poor sleep quality(OR=2.807,95%CI=1.260~6.421,P=0.012),and the ability of action were poor(OR=3.286,95%CI=1.363~8.514,P=0.010),poor appetite(OR=3.317,95%CI=1.398~8.357,P=0.008).Risk factors for depressive symptoms were age between 45-49,poor sleep quality(OR=3.948,95%CI=2.087~7.678,P<0.001),anxiety(OR=3.499,95%CI=1.654~7.627,P=0.001).Compared with those older than 55 years old,40~44 years old(OR=0.065,95%CI=0.014~0.245,P<0.001),45~49 years old(OR=0.218,95%CI=0.084~0.536,P=0.001)had lower risk of perimenopause syndrome;the degree of perimenopause syndrome is higher in those with poor sleep quality and poor ability of action.Conclusion(1)The incidence of anxiety symptoms in HIV/AIDS patients is 65.5%,which is higher than that of middle-aged and older women with similar ages in the same area. The factors affecting anxiety are family income, sleep quality, the ability of action, appetite, depressive and perimenopause syndrome.(2)The incidence of depressive symptoms in HIV/AIDS patients was 61.4%,which was higher than that of middle-aged and older women with similar ages in the same area.It was also higher than the general population in China, and HIV infection was considered as independent risk factor in depressive. The factors influencing depression were age, sleep quality and anxiety.(3)The incidence of perimenopause syndrome in HIV/AIDS patients is 85%,which is higher than that of middle-aged and older women with similar ages in the same area. The factors affecting perimenopause syndrome include age, quality of sleep and ability of action. |
