BackgroundPemphigus is a group of severe,chronic and relapsing autoimmune diseases with clinical manifestations of blisters,bullous lesions,erosions and acantholysis.Although the pathogenesis of pemphigus is currently unclear,the detection of specific Ig G autoantibodies against desmoglein-1(Dsg1)and desmoglein-3(Dsg3)in the serum and lesions of patients is considered to be closely related to the formation of skin blisters.In recent years,scholars have found through extensive research that although pemphigus is an autoantibody mediated immune skin disease,the obvious abnormality of cellular immunity is related to various pathophysiological phenomena in the pathogenesis of pemphigus,so the role of T cells and related factors in the pathogenesis is increasingly recognized.Cytokines known to be associated with pemphigus include various interleukins(IL)and tumor necrosis factor(TNF),which are mainly involved in inflammatory cell infiltration and loss of adhesion between keratinocytes.Th17 is a novel CD4+ T cell subtype discovered in recent years.Many autoimmune diseases have been reported to be connected with Th17,such as rheumatoid arthritis,systemic lupus erythematosus,multiple sclerosis and psoriasis.However,studies in pemphigus are rare,and the role of related cytokines and chemokines in pemphigus remains unclear.So,this article further examines the pathogenesis of Th17 cell-associated cytokines and chemokines in patients' serum,and further explores the occurrence of this disease.ObjectiveEnzyme-linked immunosorbent assay(ELISA)was used to measure the serum levels of cytokines IL-6,IL-17,IL-21,IL-22,IL-23 and chemokines CXCL-8,and CCL-5.To compare the levels of Th17 cytokines and chemokines in serum of pemphigus patients with different subtypes and severity.To explore the role of Th17 related cytokines and chemokines in the pathogenesis of pemphigus.Method31 cases of pemphigus admitted to the dermatology ward of the First Affiliated Hospital of Anhui Medical University from December 2017 to August 2018 were collected as case groups.the clinical,histopathological and pemphigus autoantibody tests were performed to clearly diagnose the classification and severity score.Another 20 healthy subjects were selected as the control group.Enzyme-linked immunosorbent assay(ELISA)was used to measure the serum levels of IL-6,IL-17,IL-21,IL-22,IL-23,CXCL-8,and CCL-5,and independent sample t test and variance analysis were used for comparison.ResultsThe serum levels of cytokines IL-6,IL-17,IL-21,IL-22,IL-23 and Chemokines CXCL-8,CCL-5 was significantly higher in pemphigus patients than that in normal controls(P<0.001).The differences between the three subtypes of pemphigus were no significant(P>0.05).The differences of IL-17?IL-21?IL-22?IL-23?CCL-5 were significant(P<0.05),but the serum levels of IL-6 and CXCL-8 had no difference(P>0.05)between the three severity groups.ConclusionsCytokines IL-6,IL-17,IL-21,IL-22,IL-23,and chemokines CXCL-8,CCL-5 have different degrees of elevation in the serum of patients with pemphigus,and some factors are related to the severity of the disease.Th17 correlation Cytokines and chemokines may be involved in the pathogenesis and disease state of pemphigus.
