Purpose and SignificanceVacuum-assisted lumpectomy has been widely accepted by patients with benign breast lump in many hospitals,as it generates smaller scar and could be easily operated.However,there are some postoperative complications,among which haematoma is the most frequent one.Although some haematoma can be absorbed by the body,while some needed puncture aspiration or even surgical removal,which would in turn bring about unnecessary pain and cost for the patients,and unnecessary work for the surgeons.In order to reduce the incidence of postoperative haematoma,we tried to put self-made drainage tube after surgery.This method could be easily operated and the cost of consumables is cheap,and such method has not been reported before.This study also analyzed the effect of vacuum-assisted lumpectomy plus drainage on the postoperative recovery and other complications of patients with benign breast lump.MethodsFrom January 2016 to July 2017,80 patients with benign breast lump admitted to People's Hospital of YuXi City were randomly divided into drainage group and non-drainage group by envelope method(40 patients for each group).The drainage group's patients were treated by vacuum-assisted lumpectomy combined with drainage,while the non-drainage group received vacuum-assisted lumpectomy only.The operation time,the amount of bleeding,ecchymosis area,presence of hematoma and postoperative pain were recorded.The residual effusion was evaluated 1 months after operation,the scar length,dydrops and the satisfaction degree of the patients were investigated 3 months after operation.Statistical methods were used to analyze and compare the two groups of data.ResultsThe total incidence of postoperative complications in the drainage group was 2.5%,and all of them were haematoma.In the non-drainage group,the incidence of postoperative complications,which included hematoma,infection and breast malformation,was 12%.Hematoma is the main complication in the non-drainage group,and the incidence was 7.5%.Although,the operation time of drainage group was longer than that of non-drainage group(P<0.05),the incidence of postoperative hematoma in the drainage group was significantly lower than that in the non-drainage group(P<0.05).The amount of bleeding after surgery,the ecchymosis area,the pain score and the residual cavity effusion of the drainage group were significantly lower than that in the non-drainage group(P<0.05),and the patient satisfaction rate of the drainage group also significantly higher than that of the non-drainage group(P<0.05).There was no significant difference in scar length between the two groups(P>0.05),Due to the reduction of hematoma,pain and other complications,the satisfaction of patients in the drainage group was significantly higher than that in the non-drainage group(P<0.05).ConclusionsVacuum-assisted lumpectomy combined with drainage could reduce the total postoperative complications,and the postoperative hematoma was significantly reduced,which could improve the postoperative recovery process.The disadvantage was that the operation time was slightly longer than that of the non-drainage group.The long-term effects remain to be observed.
