Background Today the success of cardiac surgery is inseparable from cardiopulmonary bypass(CPB),which plays an important role in improving the prognosis of patients.Although more and more patients are now willing to undergo cardiac surgery,the technique of extracorporeal circulation is likely to cause multiple tissue damage to the patient because of its very large stimulation to the patient.Most of the previous studies focused on organs such as the heart,lungs,and kidneys,but not enough attention to the liver.About 10% of heart surgery patients often have postoperative liver damage.Therefore,it is necessary to find a way to reduce the damage of the liver caused by CPB.Objectives This study was designed to find the hypothesis that the adenosine a2 a receptor agonist(CGS21680)could moderate hepatic injury in juvenile rats following CPB.Methods Thirty-six SD rats were distributed into three groups randomly: Sham group,CPB group and CGS21680 group.The model of CPB in young rats was established between the CPB group and the CGS21680 group(60 min of cardiopulmonary bypass with priming solution),CGS21680 was added(120?g/kg)to the priming solution before the CPB according to the direction.And the sham group underwent cannulation and heparinization only.Interleukin-1?,tumor necrosis factor-?,alanine aminotransferase(ALT),asperate transaminase(AST)and glutathione peroxidase(GSH-PX)in the plasma were measured at different time points including pre-operation(T0),30 min(T1),transfusion finished(T2)and 1 hour after operation(T3).Liver tissues were taken1 hour after operation.myeloperoxidase(MPO),interleukinin-1?,IL-6 and TNF-? were measured in the tissues,and histomorphological changes were observed by HE staining.Results In the CPB group,the levels of IL-1?,TNF-?,AST and ALT in the plasma were significantly higher than those in the sham group at different time points(P<0.05).The indices aboved in the CGS21680 group were significantly lower than in the CPB group(P<0.05).The activity of GSH-PX in the CPB group was lower than that of the sham group at different time points(P<0.05),while in the CGS21680 group was higher than that of the CPB group(P<0.05).In the CPB group,the levels of IL-1?,IL-6,TNF-? and MPO in the liver were significantly higher than those in sham group(P<0.05).Those indices were significantly lower in CGS21680 group than in the CPB group(P<0.05).Conclusions The adenosine receptor agonist(CGS21680)can ameliorate the hepatic injury caused by CPB in juvenile rats.
