Clinical Study On Treatment Of High Complex Anal Fistula By Virtual-hanging And Solid-cutting

Objective:By observing the anal function,post-operative pain,cure rate of the two surgical methods,to explore the comparative study of modified seton operation method through external sphincter approach and cut seton treatment of high complex anal fistula,so as to provide a safe and effective new surgical method for the treatment of high complex anal fistulaMethods:According to the assistant examination and intraoperative exploration,30 patients with high complex anal fistula were selected,who belonged to the Parks classification of superior and/or external sphincter anal fistulas,excluding those with secondary internal open in rectal cavity.They were randomly divided into treatment group and control group.The treatment group was treated with the method of virtual hanging and solid cutting,while the control group was treated with the traditional cut seton treatment.The total curative effect,post-operative pain score,wound healing time,quality of life score,Wexner anal incontinence score and pressure measurement were observed and recorded,and the corresponding statistical analysis was made according to the resultsResult:The therapeutic effect of high complex anal fistula was analyzed and summarized.There was no significant difference between the two groups(P>0.05).After 3 months of follow-up,there was no recurrence in the treatment group and 1 case in the control group.There was no statistical difference(P>0.05).In terms of course of treatment,there was no significant difference in wound healing time between the treatment group and the control group(P>0.05)Post-operative pain scores were scored and statistically analyzed.There was a significant difference between the two groups(P<0.05).There was no significant difference in subjective anal function score(Wexner score)and pressure measurements(anal resting pressure,anal maximum constricting pressure)between the two groups(P>0.05).The sample size of this study is small and needs further studyCONCLUSION:The virtual hanging and solid cutting treatment through external sphincter approach is similar to the traditional cut seton treatment in terms of curative effect,wound healing time,anal function,course of treatment and recurrence rate,but it has significant advantages in relieving pain after operation.
