objective:Analyze the demographic characteristics and clinical data of patients with pulmonary nodules and lung cancer detected by LDCT,and discuss the risk factors of pulmonary nodules and lung cancer,so as to provide basis for the management of pulmonary nodules and the early screening of lung cancer Data and methods:We extracted the citizen whose age are over 40 years old of 10 communities in Changchun Jilin province as target groups on April 1,2015,solstice on September 23,2015,then performed thoracic Low-dose spiral CT screening and did questionnaire survey to acquire clinical related data,then followed them up according to the results of screening.We analyzed the demographic characteristics,the exposure of related risk factors of lung cancer and the radiologic characteristics of 1435 subjects to discuss the factors which might impact the generation of lung caner or pulmonary nodule.results:1.The basic clinical data analysis of subjects :There were 574 male(40%)and 861 female(60%)of 1435 screened subjects in Changchun.The mean age was 59.86±6.73 yrs.The median BMI was 24.03(from 22.03 to 26.04).Smokers were 446(31.1%),long-term passive smoker were 403(28.1%)which contained 347 female.The female passive smoking ratio was greater than male.64.3% male were smoker as well as 91.2% female were nonsmoker,the male smoking rate was greater than female(P<0.001).There were 76 subjects(5.2%)with previous tumorous history,340 subjects(23.6%)with malignant tumorous family history,145 subjects(10.1%)with family history of lung cancer,25 subjects(1.74%)with family history of breast cancer,325 subjects(22.6%)with hypertensive history,88 subjects(6.13%)with diabetic history,22 subjects(1.53%)with bronchitic history.2.The clinical data analysis of subjects with pulmonary nodule:of all 1435 subjects of baseline screening,111 subjects with one or more than one non-calcified nodule(7.74% of all subjects).104 with solid nodule,2 with partial solid nodule and 5 with ground glass nodule.90 with solitary nodule and 21 with multiple nodule.3.The clinical data analysis of subjects with lung cancer:there were 5 subjects with lung cancer(0.35% of all subjects).4 with glandular cancer,1 with squamous cancer.4 with solitary nodule and 1 with multiple nodule.3 with solid nodule,1 with ground glass nodule and 1 with partial solid nodule.4 with lung cancer of stage?(80% of all subjects with lung cancer detected)4.The analysis of factors that might impact the incidence of pulmonary nodule or lung cancer: The proportion of smoking history,smoking index,family history of cancer and history of chronic bronchitis in the tumor group was higher than that in the undetermined nodule group(the possibility of benign nodules was higher),and the difference was statistically significant.5.For 90 patients with isolated pulmonary nodule,we carry out individualized management and follow-up according to their CT density,size and other imaging data.Among them,38 patients were re-examined according to the follow-up plan,with a compliance rate of 42.2%.The original nodules disappeared in 4 patients.The original nodules were reduced in 2 cases.Re-examination of the original nodules showed no significant changes in 27 patients.The number of reexamination nodules in the other 8 patients was higher than before,and they are still being followed up.Conclusions:1.The smoking status and secondhand smoke exposure of the subjects were consistent with the epidemiological survey results of smoking in the 2012 China tobacco control plan.2.The results of this study suggest that smoking,previous family history of cancer and previous history of chronic bronchitis may increase the risk of lung cancer.3.The malignant probability of partial solid pulmonary nodules and ground glass nodules is higher than that of solid nodules,which requires more vigilance in the early screening of lung cancer...
