Objective:To rapidly detect helicobacter pylori(Hp)infection and antibiotic resistance in Jilin area by PCR-reverse dot hybridization,and explore the main virulence gene types and the resistance background of Hp in this area.Methods:A total of 69 gastroscope samples which were tested positive by rapid urease test in the endoscopy center of the First Hospital of Jilin University were collected.The infection of Hp and the main virulence genes were analyzed,meanwhile,the resistance of Hp to five antibiotics and the distribution of mutant genes were determined by PCR-reverse dot hybridization.Result:1.Of the 69 gastric mucosa samples,40 cases were positive by PCR-reverse dot hybridization,the positive rate was 58.0%.2.Vac A virulence gene types of Hp were detected in Jilin area,which were s1/m1(42.5%),s1/m2(55.0%),s1/m1,m2(2.5%).There was no statistical difference between virulence gene and disease types(P=0.539).3.(1)Among the 31 drug-resistant strains,the resistance rate of Hp to clarithromycin was the highest(60.0%),followed by metronidazole(32.5%)and levofloxacin(30.0%),and the resistance rate of Hp to tetracycline(5.0%)and amoxicillin(12.5%)was low;(2)There were9(22.5%)Hp strains susceptible to all tested antibiotics,12(30.0%)mono-,13(32.5%)double-and 6(15.0%)triple-drug resistant strains;(3)The clarithromycin resistance gene mutation loci was A2143G(34.8%)in23S r RNA gene.The metronidazole resistance gene mutation loci was G616A(18.8%)in rdx A gene.The amino acid mutations of levofloxacin resistance gene gyr A were N81K(8.7%)and D91G/N(8.7%).The gene mutations of amoxicillin were T556S(1.4%)and N562D(1.4%)in PBP1 gene.The tetracycline resistance gene mutation mainly were AGA926-928TTC(1.4%)and AG926-927GT(5.8%)in 16 S r RNA gene;(4)There was no significant difference in the resistance rates of Hp to five antibiotics among sex,age and disease groups(all P > 0.05).Conclusion:The virulence typing of Hp vac A gene in Jilin area is mainly s1/m1 and s1/m2,but the virulence gene is not related to the type of disease.Hp resistance to clarithromycin,metronidazole and levofloxacin are high,while to tetracycline and amoxicillin was relatively low.The drug resistance was unrelated to gender,age and disease.The resistance pattern presented diversity and multiple drug resistance rates are high.The clarithromycin resistance gene mutation is A2143 G of 23 S r RNA gene,the metronidazole resistance gene mutation is G616 A in rdx A gene,the levofloxacin resistance gene are N81 K and D91G/N in gyr A gene,the amoxicillin resistance gene are T556 S and N562 D in PBP1 gene,the tetracycline resistance gene are AGA926-928 TTC and AG926-927 GT in16S r RNA gene in this area.
