Objective: By studying the expression of Tfh cells and Tfr cells in peripheral circulation of patients with Parkinson's disease,and the levels of IL-4,IL-10,IL-21,TGF-? in peripheral circulation,to investigate the role of Tfh cells and Tfr cells in the development of Parkinson's disease?Methods: We selected 40 patients with PD in the Department of Neurology,First Hospital of Jilin University from January 2017 to January 2018,while select age and gender-matched healthy controls were 30 cases.Peripheral venous blood was collected on an empty stomach in the morning in the case of informed consent of the subjects.Heparin anticoagulation were used to detect the proportion of Tfh cells and Tfr cells in CD4+ T cells in peripheral blood by flow cytometry;procoagulant were used to detect the levels of IL-4,IL-10,IL-21 and TGF-? in peripheral blood serum by Cytometric Beads Array after centrifugation.At the same time,the patient's basic information and clinical data were collected,including gender,age,and duration of disease.Parkinson's disease specialists performed H-Y staging,UPDRS-III score and NMSS score on PD patients.Finally,the expression of Tfh cells and Tfr cells in the peripheral blood of patients in the PD group and the HC group were compared,and the levels of IL-4,IL-10,IL-21 and TGF-? were also compared between the two groups.Results: 1.Compared with the HC group,the proportion of the Tfh cells in CD4+ T cells in the peripheral circulation of the PD group was significantly higher[3.68(2.64-5.70)vs 1.94(1.32-2.99),P<0.001],the difference was statistically significant.2.Compared with the HC group,percentage of the Tfr cells in CD4+ T cells in the peripheral circulation decreased,but no statistical difference[1.05(0.62-1.54)vs 1.3(0.63-1.90),P>0.05].3.Compared with the HC group,the CD19+ B cells in lymphocyte in the peripheral circulation of the PD group were significantly lower,and the difference was statistically significant[5.35(4.13-9.38)vs 8.68(5.61-12.93),P=0.014].4.There were no significant differences in the levels of IL-4,IL-10,IL-21,and TGF-? in the peripheral blood of patients with PD and HC(P>0.05).However,IL-21 levels in peripheral blood of PD patients were positively correlated with H-Y staging and UPDRS-III scores(P<0.05).Conclusion: 1.Compared with the HC group,the expression of Tfh cells in the peripheral circulation of the PD group was significantly increased,the expression of Tfr cells was not statistically different,but it had a decreasing trend.This suggests that PD patients may have a continuous activation of humoral immune response,presumably related to chronic persistent progression of PD,and may become a new target for intervention in PD disease.2.The level of IL-21 was significantly positively correlated with the H-Y stage and UPDRS-III score in the PD group,suggesting that IL-21 may be used to assess the severity of the disease.
