Comparative Research Of ERUS?MSCT And MRI In Preoperative T Staging And Circumferential Margin Detecting Of Rectal Carcinoma

Objective the value assessment of endorectal ultrasonography(ERUS)? multi-slice computed tomography(MSCT)and magnetic resonance imaging(MRI)in the preoperative T staging and circumferential resection margin(CRM)of rectal carcinoma.Methods The clinicopathological data of 200 cases of rectal cancer undergoing radical resection admitted to the first hospital of jilin university from May 2016 to May 2018 were collected.All patients underwent ERUS,MSCT and MRI examination before surgery.We analysised the differences of the coincidence rate,sensitivity,specificity and consistency in three kinds of examination by comparing the T staging and CRM with postoperative pathological results.The data were analysised by statisticai software SPSS 23.0.Results(1)the evaluation results of T stage of rectal cancer by ERUS,MSCT and MRI.The total coincidence rate by ERUS was 76.0%.The coincidence rate of T1,T2,T3 and T4 stages was 94.5%,81.0% 80.5% and 96.0%.The sensitivity was 55.6%,76.7%,79.9% and,40.0%.The specificity was 98.4%,82.2%,81.8% and 97.4%.The Kappa value was 0.511,0.616,0.583 and 0.313,P<0.05.The total coincidence rate by MSCT was 62.0%.The coincidence rate of T1,T2,T3 and T4 stages was 91.0%,80.5%,69.0% and 53.5%.The sensitivity was 0.0%,39.5%,76.1% and 100%.The specificity was 100.0%,91.7%,54.5% and 83.1%.The Kappa value was0.000,0.351,0.304 and 0.197,P<0.05.The total coincidence rate by MRI was 67.5%.The coincidence rate of T1,T2,T3 and T4 stages was 91.0%,75%,75.5%and 94.5%.The sensitivity was 0.0%,46.5%,84.3% and 60%.The specificity was 100.0%,82.8%,57.6% and 95.4%.The Kappa value was 0.000,0.284,0.431 and 0.329,P<0.05.(2)evaluation of CRM in rectal cancer by ERUS,MSCT and MRI.The sensitivity,specificity,coincidence rate and Kappa value of ERUS were 100.0%,97.4%,97.5% and 0.655.The results of MSCT were 60.0%,93.8%,93.0% and 0.273.The results of MRI were 80.0%,95.4%,95.0% and 0.424.P=0.000.Conclusions ERUS examination has the best coincidence rate of preoperative T staging and CRM evaluation for rectal cancer,which is suitable for accurate preoperative staging of rectal cancer and can provide a reliable basis for the selection of treatment schemes and surgical methods.For T staging evaluation,ERUS showed the best effect for T1 and T2 tumors.For T3 stage tumors,MRI has certain advantages.For T4 stage tumors,MSCT has certain advantages.In clinical practice,preoperative evaluation of MSCT+ERUS+MRI is recommended,and in the case of MRI limitation,preoperative evaluation of MSCT+ERUS is recommended.
