Objective:To understand the status of cerebrovascular function and the exposure level of Cerebral Vascular Hemodynamic Index?CVHI?among different sex and age groups in the physical examination center of the Affiliated Hospital of Guizhou Medical University.To explore the correlation between cerebral hemodynamics and arteriosclerosis index,Brachialal Ankle Pulse Wave Velocity?BaPWV?.To further clarify the clinical significance and reference value of CVHI score in stroke risk prediction,so as to provide scientific basis for the prevention and treatment of cerebrovascular diseases.Methods:A cross-sectional survey was conducted among 18279 people who volunteered to take part in medical examinations at the Medical Examination Center of Affiliated Hospital of Guizhou Medical University from September 2017 to September 2018.According to the inclusion and exclusion criteria,13948 people who met the requirements were selected and the following survey:?1?The collection of baseline data includes General Information,Past History,Systolic Blood Pressure,Diastolic Blood Pressure,Blood Lipid,Fasting Blood Sugar,Glycosylated Hemoglobin,Uric Acid and Homocysteine.?2?Carry out the detection of arteriosclerosis and cerebrovascular function.According to the cerebral hemodynamics integral value,the physical examination population was divided into cerebrovascular health group and stroke risk group.The risk group was divided into three risk subgroups according to the CVHI integral value,and the differences of clinical indicators among different groups were compared.To explore the change of CVHI score in different sex and age groups and the proportion of abnormal score?CVHI<75?.The second part:The measured BaPWVs were grouped according to the quartile method,and the distribution of clinical indicators,especially CVHI integral values,among the four groups was discussed.To analyze the change of CVHI integral value and the change of BaPWV in different genders,and to determine whether there is risk of stroke?CVHI score?75or CVHI score<75?as the two-class dependent variable,multi-factor binary Logistic Regression analysis was performed to investigate the correlation between CVHI scores and BaPWV values and other risk factors for stroke.The original data will be collected and the database will be established in the form of double check input.SPSS 20.0software is used for data analysis.The statistical methods involved in this study include descriptive analysis,independent sample t-test,one-way ANOVA,Chi-Square Test,Chi-Square Segmentation,SNK method,Non-Parametric Kruskal-Wallis H test,Nemenyi method and Multi-Method.Factor binary Logistic Regression analysis.The bilateral test level was?=0.05.Results:The first part:?1?The study finally included13948 cases,8278 males and 5670 females.There were 1883 people with risk of stroke,accounting for 15.6%of the total research population.There were significant differences in age and CVHI scores between the cerebrovascular health group and the risk group?P<0.05?.?2?Age,weight,height,BMI,systolic blood pressure,diastolic blood pressure,fasting blood glucose,glycated hemoglobin,total cholesterol,triglyceride,homocysteine,uric acid,brachial-ankle pulse wave velocity?BaPWV?.There were significant differences among the four groups?P<0.05?.?3?The CVHI integral value generally shows a downward trend with age,and the proportion of abnormal integral value increases with age.The second part:?1?The difference of BaPWV in gender group was statistically significant?P<0.05?,the pulse wave conduction velocity of male was faster than that of female in general.?2?The difference of BaPWV between male and female was significant?P<0.05?.The pulse wave velocity of male was faster than that of female.?3?After grouping the measured BaPWV quartiles,baseline indicators:age,height,weight,BaPWV,BMI,systolic blood pressure,diastolic blood pressure,fasting blood glucose,glycosylated hemoglobin,total cholesterol,triglycerides,high-density lipoprotein cholesterol The values of low-density lipoprotein cholesterol,homocysteine,uric acid and CVHI were statistically significant between the Q1Q4 groups?P<0.05?.?4?Multivariate binary logistic regression analysis showed that obesity,hypertension,diabetes mellitus and atherosclerosis in different degrees were significantly correlated with cerebral hemodynamic parameters.Moderate to severe atherosclerosis?30%-50%higher than normal BaPWV threshold?had the highest correlation with CVHIV score,OR=6.076,95%CI?4.895,7.542?.Conclusion:?1?The exposure of different stroke risk groups in different gender,age groups and various basic clinical indicators was different,and the difference was statistically significant.?2?With the increase of age,vascular aging,CVHI score decreased,abnormal score rate increased,and the risk of stroke increased.?3?In the middle-aged and elderly women after the age of 55,the health status of cerebrovascular function decreased significantly,which was more than that of men.?4?The velocity of brachial and ankle pulse wave is faster in males than in females.?5?CVHI score decreased,BaPWV increased,vascular stiffness increased and vascular compliance decreased.?6?The highest correlation was found between the risk of stroke and Q4 group of BaPWV.?7?Through this study,targeted health management can be put forward for the high-risk population of stroke.
