Objective: Most long-distance amateur runners suffer from increasing high risks that acute sports injuries transform into chronic because the lack of professional guide for running technology and correct sports rehabilitation post injuries.The main purpose of this study is to investigate whether hip muscle strengthening combined with gait retraining has better both short and long term effect in anterior knee pain,lower extremity function,hip abduction peak torque and lower extremity alignment.So as to provide guidance for treatment of patellofemoral pain syndrome and prevention of patellofemoral osteoarthritis.Methods: Forty female patellofemoral pain patients with excessive hip adduction during running were randomly divided into hip muscle strengthening group,gait retraining group,hip muscle strengthening combined with gait retraining group and control group.Subjects in the experimental group underwent four-week,three-week(12 times)intervention training to observe knee pain,lower extremity function,hip abduction peak torque and lower extremity alignment before,immediately after,and one month after intervention.Results:(1)There was no significant improvement in hip function and hip rotation angle during running(P>0.05),but knee joint pain,other lower extremity alignment indicators and hip abduction peak torque.There was a short-term improvement(P<0.01)and the improvement of knee pain was maintained for a long time(P<0.01).(2)Gait retraining alone for knee pain(P<0.05)and lower limb function(P<0.01)and the lower extremity alignment(P<0.01)was improved and all were maintained for a long time(P<0.01),but there was no significant improvement in hip abduction peak torque(P>0.05).(3)Hip muscle strengthening combined with gait retraining improved all the experimental indicators and maintained for a long time(P<0.01).(4)In terms of the angle of the hips during the single leg squat(P<0.05)and the hip rotation angle(P<0.01)and the hip adduction angle during running(P<0.01),hip muscle strengthening combined with gait retraining improved more significantly in short-term than hip muscle strengthening alone;(5)As for hip adduction angle during running,gait retraining alone is better than hip muscle strengthening alone in short-term(P<0.05),hip muscle strengthening combined with gait training had good long-term efficacy(P <0.01)compared to gait retraining alone.Conclusions:(1)Both hip strength training and gait retraining can be used to improve the knee pain symptoms of female runners with patellofemoral pain syndrome.(2)Hip Weekly strength training combined with gait retraining is better for the female runners with patellofemoral pain syndrome.(3)The effect of hip strength training and gait retraining alone is different,according to The patient's clinical symptoms are targeted to select one or a combination of treatments to improve recovery efficiency.
