Study On Resource Allocation And Equity Problem Of Community Health Service In Luoyang City

General Secretary Xi Jinping emphasized that "putting people's health level on the strategic position of giving priority to development and guaranteeing people's health in an all-round and full-cycle way",which is not only the goal of the reform of China's medical and health system,but also the future development direction of medical and health services under the strategic background of healthy China.The government will put the establishment and improvement of the health service system in Du District into the focus of the work of building the grass-roots health service system in the whole society,accelerate the construction of the urban community health service network,so that it can assume the responsibility of the "gatekeeper" role of residents' health.Therefore,it is of great practical significance to study the problems and fairness in the allocation of community health service resources,optimize the allocation of community health service resources based on the research results,and provide objective basis for the government and health administration departments to formulate relevant policies,so as to realize that everyone has access to basic medical and health services and improve the health level of the whole people.The theme of this paper is the fairness and problems of resource allocation of community health services in Luoyang City from 2013 to 2018,and the possible reasons for the current situation are analyzed,and some suggestions are put forward.The research object of this paper is all community health service institutions in six districts of Luoyang City,including community health service centers and community health service stations.The theoretical tools used are public goods theory,health resource allocation theory and health regional planning theory.The quantitative tools used to evaluate fairness are Gini coefficient,Theil index,its decomposition index and concentration index.According to the needs of the calculation of Theil index,the six districts of Luoyang City are clustered according to the availability of data and the rationality of clustering results,and the results are divided into Jianxi District of underdeveloped areas according to the two indicators of per capita GDP and per capita disposable income.Xigong District and Old Town;the Hui and Luolong districts with more developed economy;the Jili District with more developed economy.Based on descriptive statistical analysis of the data of resource allocation of community health services in Luoyang City from 2013 to 2018,this paper calculates the equity of health human resources of all community health service institutions in Luoyang City: health technicians,practicing(assistant)physicians and registerednurses;material resources: number of institutions,number of beds;financial resources:financial subsidies,totaling six indicators.The results show that the equity of community health service resources in Luoyang is better than that in geographical distribution according to population distribution,the contribution rate of inter-regional equity to overall equity is greater than that in intra-regional equity,and the existence of health resources is conducive to the inequity of underdeveloped areas.Accordingly,the following suggestions are put forward: formulating good policies to attract high-quality health human resources;improving the infrastructure construction of community health service institutions;increasing investment in funds,improving the compensation mechanism;increasing publicity efforts to enhance the trust of residents in community health care.
