| Background:Stroke is one of the most common cerebrovascular diseases,and post-stroke Epilepsy(PSE)refers to the seizure caused by the stroke,which is a common complication of stroke.Seizures not only affect the course and prognosis of stroke,but also increase the disability rate of patients,which has a certain impact on the later life quality of patients.Therefore,it is of great significance to treat epilepsy after stroke.Traditional Chinese medicine(TCM)has been proved to be effective in the treatment of stroke and epilepsy,so for the treatment of PSE,whether TCM can play its role deserves further investigation.Neuroinflammatory reaction is closely related to ischemic brain injury and has been shown to be the essential mechanism in the pathophysiology of epileptic brain.Therefore,cytokines can be used to evaluate brain injury.In this study,traditional Chinese medicine combined with western medicine was used for the treatment of PSE,to study the efficacy of traditional Chinese medicine,and explore the possible mechanism of traditional Chinese medicine in the treatment of ischemia PSE with phlegm and blood stasis,to further provide a theoretical basis for TCM treatment of PSE.Objective:To investigate the curative effects of Huatan Tongluo Decoction and Ding Xian Wan with Oxcarbazepin in ischemia PSE with phlegm and blood stasis and its effect on cytokines,analyze the possible mechanism of action.Methods:To choose 80 PSE patients who up to standard.According to random number table,divided the patients into the control group and the treatment group with 40 cases each,control group(n=40)was treated with Oxcarbazepin,and treatment group(n=40)was treated with Huatan Tongluo Decoction and Dingxian Wan except for Oxcarbazepin,the treatment course was 3 months.Before and 1,3months after treatment,patients were scored with TCM syndrome score and NIHSS,and the tumor necrosis factor(TNF)-α,interleukin(IL)-1β,interleukin-6(IL-6)levels were detected.The number of seizures in the first and third months after treatment was recorded.24-hour video Electroencephalogram(EEG)was detected after 3 months.Adverse reactions of patients during treatment were recorded.Result:(1)After 1 month of treatment,the scores of TCM symptoms in both groups decreased(P<0.05),and the treatment group decreased significantly compared with the control group(P<0.05).After 3months of treatment,the scores of TCM symptoms in the treatment group decreased significantly compared with that in the first month,(P<0.05),while the scores of TCM symptoms in the control group did not decrease significantly(P>0.05),the effective rate of TCM symptoms treatment in the treatment group was better than that in the control group.(2)After 1 and 3 months of treatment,the NIHSS score in both groups decreased(P<0.05),and the reduction degree of NIHSS score in the treatment group was greater than that in the control group(P<0.05).(3)The levels of TNF-α,Il-1β,and Il-6in the two groups decreased after 1 and 3 months of treatment(P<0.05),and the levels in the treatment group were lower than those in the control group(P<0.05).(4)There was no significant difference in the number of seizures between the two groups at the first month of treatment(P>0.05),but the number of seizures in the treatment group at the third month of treatment was less than that in the control group(P<0.05).The 24-hour video EEG showed that after 3 months of treatment,the epileptic discharge in the treatment group was significantly less than that in the control group(P<0.05).(5)There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Huatan Tongluo Decoction and Ding Xian Wan with Oxcarbazepin in the treatment of ischemia Poststorke Epilepsy with phlegm and blood stasis,can enhance the control effect on seizures,decrease the number of seizures and epileptiform discharge,it also can promote the recovery of neural function in patients with ischemic stroke,significantly reduce the levels of TNF-α,Il-1β,and Il-6,compared with the use of Oxcarbazepin alone,plays a role of comprehensive treatment and improves clinical efficacy.The mechanism may be related to the reduction of cytokine levels in the serum of patients.And no obvious adverse reactions are found in the clinic,which is safe and effective and worthy of clinical promotion. |
