Due to various factors such as environment,diet,work schedule and population aging,the incidence of malignant tumors has increased year by year,which has become one of the main factors threatening human health,causing huge disease burden and economic pressure to the world.At present,there are mainly the following methods for the treatment of tumors:surgery,chemotherapy,radiotherapy and immunotherapy.These treatments have their own specificity and application,but they also have some defects.In this paper,combined with the research direction and actual situation of our research group,we have selected two kinds of tumor treatment methods,chemotherapy and surgery,which are the most widely used at present,and conducted some research.Chemotherapy is a means of systemic treatment for patients.Chemotherapy drugs reach most of the body's organs and tissues through circulation,and have a good effect on killing metastatic cancer cells and tiny tumor metastases.However,while chemotherapeutic drugs kill tumor cells,they also have a great lethal effect on normal cells that are rapidly dividing in the human body,leading to strong side effects such as digestive tract reaction and hair loss in cancer patients.These side effects also limit the amount of chemotherapy drugs to some extent.At the same time,the function and structure of abnormal vascular and the high interstitial pressure in the tumor also limit chemotherapy get into tumors.The bioavailability of chemotherapy drugs is low and the chemotherapy effect is poor.In response to this situation,we have developed a chemotherapy-assisted drug.We have found that using HPSE inhibitors to reduce the expression level of HPSE in tumors can improve the structure and function of blood vessels in tumors,and tend to normalize them,thereby improving the delivery efficiency and bioavailability of chemotherapeutic drugs.In turn,the effect of chemotherapy is improved.Improving the delivery efficiency and bioavailability of chemotherapeutic drugs can reduce the amount of chemotherapeutic drugs,reduce side effects,and improve the life quality of patients under the premise of ensuring therapeutic efficacy.This part of our study provides a theoretical basis for the use of HPSE inhibitors as an adjunct to chemotherapy.Surgical treatment is the most common treatment in clinical practice.Direct removal of tumor lesions by surgery can effectively improve the survival of patients.However,in addition to the difficulty in completely removing the lesions during the operation,a very serious problem is faced,that is,whether lymph node metastasis occurs around the lesion.Whether lymph node metastasis happened is an important and independent factors affecting the prognosis of cancer patients.In order to find and clear metastatic lymph nodes as much as possible,the predecessors had made a lot of exploration,including relying on the surgeon experience,mucosal staining,radionuclide labeling Methods,intraoperative ultrasound imaging and rapid pathology etc.,but these methods have their own defects.Therefore,image navigation technology that can be imaged in real time during surgery has become a research hotspot.In this study,we rely on the self-established large animal model,the rabbit cervical lymph node high metastasis model,to study the problems currently existing.We used two kinds of fluorescent tracers—ICG and 5-ALA to improve the accuracy of intraoperative detection of metastatic lymph nodes in real time.To some extent,we overcome the high false positive rate and high false negative rate of single tracer.Through this study,we have made some contributions to the application of image navigation technology in cancer therapy.At the same time,we also analyzed the biological parameters of lymph nodes,and initially analyzed the causes of false positives and false negatives when ICG differentiated lymph nodes.It was found that the fluorescence intensity of lymph nodes had a strong correlation with the density of lymphatic vessels in lymph nodes.
