Objective:To investigate the expression level and significance of IL-35 in serum,bronchoalveolar lavage fluid of pulmonary tuberculosis and tuberculous pleural effusion.Methods:The subjects recruited 33 cases of TPE(14 cases of simple TPE),27 cases of MPE;31 cases of pulmonary tuberculosis,22 cases of lung cancer who were hospitalized at First Affiliated Hospital of Guangxi Medical University and Fourth People's Hospital of Nanning from January 2016 to December 2017.Peripheral venous blood serum,supernatant of pleural effusion and BALF were collected from the subjects.IL-35 expression in peripheral blood serum,pleural effusion,and lavage fluid was measured by double sandwich enzyme-linked immunosorbent assay(ELISA).The data obtained in this experiment were all imported into SPSS22.0 statistical software for statistical analysis.Results:(1)Pleural effusion: the concentration of IL-35 in the TPE group was significantly higher than that in the MPE group(P<0.05).(2)BALF:the concentration of IL-35 in the PTB group was significantly higher than that in the LC group and TPE group,respectively(P<0.05);there was no significant difference between TPE group and LC group(P>0.05).(3)Serum:the concentration of IL-35 in the TPE group was significantly higher than that in the PTB group(P<0.05);the concentration of IL-35 in the MPE group was significantly higher than that in the LC group(P<0.05);there was no significant difference between the PTB group and LC group.(P>0.05);there was no significant difference between TPE group and MPE group(P>0.05).(4)Pleural effusion and corresponding serum:the concentration of IL-35 in the pleural effusion of the TPE group or MPE group was significantly higher than that in the corresponding serum,respectively(P<0.05).(5)BALF and corresponding serum:the concentration of IL-35 in the BALF of the PTB group was significantly higher than that in the corresponding serum(P<0.05);the concentration of IL-35 in the serum of LC group or TPE group was significantly higher than that in the corresponding BALF,respectively(P<0.05).(6)BALF and corresponding pleural effusion:the concentration of IL-35 in the pleural effusion of TPE group was significantly higher than that in the corresponding BALF(P<0.05).(7)Correlation: there was a positive correlation between the concentrations of IL-35 in the BALF and serum of PTB group(correlation coefficient r=0.403,P<0.05);there was no correlation in any two of the concentrations of IL-35 in the serum or corresponding pleural effusion,BALF of the other groups.(8)ROC curve analysis: the optimal threshold for IL-35 concentration in diagnosis of pulmonary tuberculosis in BALF was 177.6 pg/ml,which's sensitivity was 0.710,and specificity was 0.773.The optimal threshold for IL-35 concentration in the diagnosis of tuberculous pleurisy in pleural effusion was 62463.7 pg/ml,which's sensitivity was 0.848,and specificity was 0.630.Conclusion: 1.IL-35 is elevated in bronchoalveolar lavage fluid of pulmonary tuberculosis and pleural effusion of tuberculous pleurisy,which may be involved in the immune response of local tuberculous lesions.2.IL-35 in pleural effusions is less valuable in the diagnosis of tuberculous pleural effusions;IL-35 in bronchoalveolar lavage fluid is expected to be a new indicator for the diagnosis of pulmonary tuberculosis.
