ObjectivesAccording to the survey and analysis of supply and demand of pregnant women and medical staffs,an informationized health education program was formed during pregnancy, and then an informationized health management mode was constructed during pregnancy. To explore the impact of this model on pregnant women's compliance of perinatal care, maternal and child health literacy, pregnancy outcome and satisfaction and evaluate its effect.MethodsThis research included three parts:1.The formation of informationized health education programs for pregnant women.Firstly,145 cases of pregnant women and 135 cases of obstetric medical staff in 3 hospitals form Henan province were selected to investigate and analysis their supply and demand of health education during pregnancy,to provide a basis for the formation of information-based health education program during pregnancy.Secondly, based on the effeces of literature review and supply and demand survey, the research team formed the first draft of information-based health management program. Finally, the final draft of the informationized health education program for pregnant was revised by the expert group meeting.2.Construction of information-based health management model during pregnancy and delivery.Based on the final draft of the informationized health education program,a tertiary prevention management system was established,a health management team was built,a health management process was formulated, the informationized platform interface was set up,the port of pregnant women and the port of medical were opened, the informationized health management platform was set up,Finally,a new model of informationized health management during pregnancy and delivery was formed.3.Application and evaluation of information-based health management model in pregnant and perinatal period. It is convenient to select 110 pregnant women in the outpatient department of a general hospital in Zhengzhou City as the research objects. The pregnant women whose first enclosing date was odd number entered the intervention group and another entered the control group. In the control group, the routine method of perinatal care was adopted. The intervention group applied the informationized health management platform for pregnant and parturient women on the basis of routine perinatal care, and the pregnant women were given full gestational weeks and seamless systematic health management. The platform was evaluated for effectiveness by using the Maternal and Child Health Literacy Survey Form, the Health Education Job Satisfaction Scale, pregnancy outcomes, perinatal care compliance, and semi-structured interviews with pregnant women in the intervention group.Data were analyzed with SPSS Statistics 21.0,using the descriptive statistical analysis,T test,chi-square test,Fisher's exact test, repeated measurement of variance analysis, and generalized estimation equation.P<0.05 was considered statistically significant.Results1.The informationized health education programs for pregnant women is divided into two parts:content and function. The content is divided into regular health management and the management of complications during pregnancy, and regular health management including prenatal, nutrition, rest and activity, and expert consensus, authoritative guide interpretation six modules, pregnancy complications management increased disease knowledge, medication guidance and management of specialized subject doctor, etc. In terms of functions,it includes eight modules: my health record(data collection),obstetric examination reminder(protection plan),small class during pregnancy(health intervention),baby's fetal movement(dynamic monitoring),my pregnant mother's circle(medical record search),health consultation(doctor-patient communication),activity appointment(activity planning)and assisting scientific research.2.According to Nationnal Health Commission of the People's Republic of China issued the "Maternal Pregnancy Risk Assessment and Management Norm",the pregnant women will be given five colors, and three levels of management, forming an information-based health management model for pregnant women and realizing the whole-process,efficient and individualized health management for enrolled pregnant women.3.A total of 110 subjects were included in the study,4 cases were lost, the loss rate was 3.64%,and the effective sample size was 106 cases, including 53 cases in the intervention group, mean age(27.40±4.14),53 cases in the control group, mean age(28.02±4.08).(1)Comparison of perinatal care compliance between the two groupsFisher's exact probability method showed that there was a statistically significant difference in perinatal care compliance between the two groups(P<0.05).(2)Comparison of maternal and child health literacy scores before and after interventionThe results of repeated measurements of analysis variance indicated that the total scores of the maternal and child health literacy of the group effect, time effect, and interaction between the group effect and time effect between two groups was statistically significant at each time point(P<0.05);the scores of the basic knowledge Results and concept dimension,of the time effect and interaction between the group effect and time effect between two groups was statistically significant at each time point(P<0.05);the scores of the healthy lifestyle and behavioral dimensions of the group effect, time effect, and interaction between the group effect and time effect between two groups was statistically significant at each time point(P<0.05);The results of generalized estimation equation indicated that the scores of the basic skill dimension of the time effect was statistically significant at each time point(P<0.05).(3)Comparison of pregnancy outcomes between the two groups?2 test or Fisher's exact probability method showed that there was no significant difference in the gestational age, mode of delivery, postpartum hemorrhage, and complications during pregnancy between the two groups(P<0.05).The birth rate of the abnormal weight of the intervention group was low.In the control group, the difference was statistically significant(P< 0.05).(4)Comparison of the level of satisfaction between the two groupsThe results of paired sample t-test showed that there was significant difference in the satisfaction between the two groups before and after intervention(P<0.05).The results of independent sample t-test showed that the difference of satisfaction between the two groups was statistically significant(P<0.05).Conclusion1.Based on the hospital platform, the Informationized health management model which focusing on the demands of pregnant women and medical staffs, achieve the pregnant women whole pregnancy, classification, efficient health management.2.Using the information-based health management model during pregnancy to manage pregnant women can effectively enhance the perinatal health care compliance, improve maternal and child health literacy level and satisfaction.Although the effect is not obvious in improving the pregnancy outcome, still haing a positive effect.
