BackgroundGastric cancer is a common digestive tract tumor in China,And then it is a long-term public health problem in China.With the wide application of radical gastrectomy,the survival of patients is prolonged,but the postoperative complications still affect the health of patients.Pleural effusion is one of the common complications to postoperative gastric cancer,which affects the postoperative rehabilitation of patients.At present,there are still few studies on postoperative pleural effusion in gastric cancer.ObjectiveTo analyze the associated factors of pleural effusion after radical gastrectomy in order to guide clinical measures to reduce the incidence of pleural effusion.MethodsWe retrospectively analyzed the clinical data of 312 patients who underwent radical gastrectomy for gastric cancer in the First Affiliated Hospital of Zhengzhou University from May 2017 to May 2018.Univariate analysis of pleural effusion and postoperative hospital stay was performed.The indicators such as operation time,surgical procedure,and resection range were analyzed by single factor analysis,and statistically significant indicators were used for multivariate logistic regression analysis.Result1.The occurrence of pleural effusion is significantly associated with prolonged postoperative hospital stay in patients with gastric cancer(P<0.05).2.Univariate analysis showed that age,gender,resection range,preoperative serum albumin level were not associated with pleural effusion after radical gastrectomy(P>0.05);laparoscopic surgery,operation time,postoperative serum albumin level,intraoperative fluid input and intraoperative blood loss were associated with pleural effusion after radical gastrectomy(P<0.05).3.Multivariate analysis showed that operative time >4h,laparoscopic surgery and postoperative serum albumin>30g/L were independent factors influencing pleural effusion after radical gastrectomy(P<0.05).ConclusionPleural effusion after radical gastrectomy is common and occurs 4-7 days after surgery,and compared to subtotal gastrectomy,maybe total gastrectomy lead to a higher incidence of pleural effusion.Laparoscopic surgery and maintenance of good serum albumin levels after surgery can effectively reduce the incidence of pleural effusion;For patients with longer operation time,we should be vigilant and give timely treatment to reduce the occurrence of postoperative pleural effusion.
