The Multiple Regression Analysis Of The Pleural Effusion After Hepatectomy

To analyze the risk factors and incidence rate of pleural effusion after hepatectomy, to investigate possible measure to decrease incidence rate of pleural effusion of hepatectomy.MethodThis study retrospectively analyzed the cases of the particular disease that the liver sugeries were operated from January 2006 to January 2009,292 Patients, which had the integrated data means. We use the SPSS17.0 to analyze the factors of pleural effusion of hepatectomy, which included age,sex, hepatic segments of lesion, size of tumor, block time of porta hepatis, bleeding of traoperative, cirrhosis,pressure of portal vein, potein level after hepatectomy.First, we divde these factors to sevel groups,use the Digital Code to take the place of the factors; secondly,we use the single factor analysis to slecet the relevant influencing factor; thirdly,we use the method of logistic analysis to exorcize confounding factors and slecet independent influential factor.ResultAmong 292 patients,55 pstients who had pleural effusion, the incidence rate of pleural effusion is 18.84%.none need thoracentesis; 170patients of primary liver cancer, 4patients of secondary hepatic carcinoma, 100patients of hepatic haemangioma,5 patiets of atypia hyperplasia,4 patients of follicular dendritic cell tumor,9 patients of FNH, the number of pleural effusion in these patients is 34,0,21,0,0,0, seperatly there is no relation between pleural effusion and the nature of primary disease; but age, sex, hepatic segments of lesion, size of tumor, block time of porta hepatis, bleeding of intraoperative, cirrhosis, potein level after hepatectomy were the influencing factors; analytically hepatic segments of lesion, blocking time of porta hepatis, bleeding of intraoperative, cirrhosis, potein level after hepatectomy is the dependent factors.DiscussionInfluencing factors of hepatectomy are varietal, analytically hepatic segments of lesion, blocking time of porta hepatis, bleeding of intraoperative, cirrhosis, potein level after hepatectomy is the dependent factors.To reduce the incidence rate of ural effusion:①Control indication strictly.②Hemostasis strictly intraoperative,to prevent the damnification of Ischemia and reperfusion injury。③It is best to short blocking time of porta hepatis,④Avioding mobilize the ligament of the liver.⑤Maintian the level of albumin.
