Objective:Through epidemiological analysis of clinical data of HFMD patients in Binzhou from 2010 to 2015,the purpose is to master the epidemic characteristics of HFMD in this area.Meta-analysis was conducted on the risk factors of severe HFMD.To explore the risk factors of severe HFMD in Binzhou.Provide a scientific basis for the prevention and control of HFMD epidemic and the early identification of severe cases in this area.Method:Direct descriptive analysis of the case data of HFMD patients directly reported by the.Binzhou Medical University Hospital from January 1,2010 to December 31,2015 was conducted, which mainly included age, gender, occupation, time of onset, severity of disease, cases Source,to summarize Binzhou HFMD epidemiological characteristics.Retrieval Publications published January 1,2012 to December 31,2017 on the risk factors for severe HFMD,for fever, maximum body temperature over 38.5?,fever over 3 days, vomiting, convulsions, mental status,EV71 serum Antibodies, whether low-level hospital first visit, caregivers of a total of nine factors for Meta-analysis.According to the 1:1 matched case-control study in epidemiology, univariate analysis and multivariate conditional logistic regression analysis were used to screen the risk factors of severe HFMD in Binzhou.ROC analysis was performed on the relevant risk factors to evaluate their diagnostic value. Results:From January 2010 to December 2015,there were 14,385 cases of hand-foot-mouth disease diagnosed and treated in outpatient department and ward of Bin zhou Medical University Hospital.According to the year, there were 2423 case s,3191 cases,1765 cases,2788 cases,2684 cases,994 cases,223 cases(1.55%).The peak incidence peaked in June(accounting for 29.75%).The age of onset of children under 5 years of age(86.94%),male incidence was higher th an that of women(1.61:1).Hand-foot-mouth disease patients mainly in out-pati ent treatment(89.04%).This study involve 2982 cases,of which 1172 were in the case group and 1810 in the control group.Meta-analysis showed fever,th e highest body temperature over 38.5 ?,fever more than 3 days, vomiting, conv ulsions, poor spirit, positive EV71 serum antibody,low-level hospital first visit, car egivers with low educational level as a risk factor for severe hand-foot-mouth disease. In the analysis of risk factors of severe HFMD in Binzhou, the influe ncing factors of HFMD were screened out by univariate analysis, which were r esidential tylpe, delayed treatment, first visit of low-grade hospital,fever,fever?38.5 ?,fever days?3days,oral ulcers, vomiting, poor spirits, convulsions, whi te blood cells>10X 10^9/L,blood glucose>6.10mmo/L,EV71 serum antibody posit ive,EEG extensive abnormalities.Multivariate logistic analysis showed delayed treatment, vomiting, poor spirits, convulsions, blood glucose 6.10mmo/L,as a ri sk factor for severe HFMD.ROC analysis of screening factors, mental retardati on and convulsions have a higher diagnostic value. Conclusion:From January 2010 to December 2015,there was a general epidemic of HFMD in Binzhou.Male children under the age of 5 were the predominant susceptible population, and health education and interventions should be stepped up to key populations by June each year.Outpatient and home isolation is the key to controlling HFMD.Binzhou HFMD incidence of mild disease, the incidence of severe low.Patients with delayed treatment, frequent vomiting, poor psychosis, limb involuntary convulsions and peripheral blood glucose, suggesting that the rapid development of severe high risk.Clinically, the epidemic characteristics of HFMD should be accurately grasped and early comprehensive and effective prevention and control measures should be taken to curb the outbreak of HFMD,reduce the incidence of severe cases and reduce the burden of disease.
