Objectives To explore the relationship between anxiety and depressive symptoms,resilience and rumination of HIV/AIDS prisoners in a prison of shandong province,so as to provide theoretical basis for improving the status quo of anxiety and depressive symptoms of HIV/AIDS patients,improving their life quality and treatment compliance,and promoting the active participation of prisoners in the reform.Methods HIV/AIDS patients were chosen from a prison in Shandong Province by adopting convenience sampling methods in this survey.The survey tools include general information questionnaires,the Hospital Anxiety and Depression Scale(HADS),Connor-Davidson Resilience Scale(CD-RISC-10),Ruminative Responses Scale(RRS).T test,variance analysis,Pearson correlation analysis,and multivariate linear regression analysis are applied to explore the relationship between resilience,ruminant and anxiety and depressive symptoms via SPSS.Results1.General demographic and disease information status of HIV/AIDS patients among prisoners:Among 215 prisoners with HIV/AIDS,the ages ranged from 18 to 63,with an average age of(33.82±8.24)years old.There were 207 male patients,accounting for 96.3%of the total number.The number of han people accounts for 56.7%,while the member of people from other ethnic groups accounts for 43.3%.53.0%of the prisoners had only primary school education or even lower education background.48.4%of the respondents were unmarried.54.9%of the subjects had been in prison for 1 year or less.36.3%of the subjects had been infected for less than 1 year and 42.3%had been infected for 1 to 5 years.40.9%of people were infected with HIV due to drug abuse.2.The anxiety symptoms of HIV/AIDS patients:The anxiety scores of HIV/AIDS patients ranged from 1 to 21,with an average score of(7.92±3.33),and the depression scores ranged from 0 to 21,with an average score of(6.99±3.80).Compared with the results of counterparts in general hospital internal medicine outpatients,the anxiety scores and depression scores of HIV/AIDS patients in this study were higher than the norms.The difference was statistically significant(all P<0.001)3.The score of anxiety and depressive symptoms of HIV/AIDS patients among prisoners and its analysis on general data:the total scores of HADS of the study subjects ranged from 2 to 36 points,with an average of(14.91±6.34)points.The scores of anxiety subscale ranged from 1 to 21,with an average score of(7.92±3.33).The scores of depression subscale ranged from 0 to 21,with a score of(6.99±3.80).The positive rate of anxiety and depressive symptoms was 54.4%and 48.4%respectively.T-test or anova results showed that there were differences in depressive symptoms among patients with different education levels(P=0.021).The positive rate of depression was higher in minority patients,and the difference was statistically significant(P=0.013).The chi-square test results showed that patients with high school education or technical secondary school had a higher positive rate of depression(P=0.014).Patients infected with HIV through drug use had a higher positive rate of depression,and the results were statistically significant(P=0.028).There was no statistically significant difference between anxiety scores and anxiety positive rate in demographic and disease information.4.Comparison of resilience and rumination scores between the negative and positive groups of anxiety and depression:the resilience scores of the positive group of anxiety were significantly lower than those of the negative group of anxiety,and the total scores of rumination,forced thinking,symptom rumination and introspection were significantly higher than those of the negative group of anxiety,with statistical significance(all P<0.001).The resilience score of the depression positive group was significantly lower than that of the depression negative group,and was significantly higher than that of the negative group in rumination total score,forced thinking and symptom rumination(all P<0.001),but there was no statistical difference in introspection(P=0.061).5.Correlation analysis of resilience,rumination and anxiety and depressive symptoms in HIV/AIDS patients in prison:Pearson correlation analysis results showed that there was a negative correlation between anxiety symptoms and resilience,with a correlation coefficient of-0.279(P<0.001).There was a positive correlation between anxiety symptoms and rumination dimensions and total rumination score,and the correlation coefficient was 0.332?0.574(all P<0.001).There was a negative correlation between depressive symptoms and resilience,with a correlation coefficient of-0.486(P<0.001).Depressive symptomswas positively correlated with compulsive thinking,symptom rumination and total score of rumination,with correlation coefficients of 0.281,0.463 and 0.378,respectively(all P<0.001).6.Regression analysis of factors influencing anxiety and depressive symptoms in HIV/AIDS patients among prisoners:anxiety symptoms was used as the dependent variable,and resilience and rumination as independent variables for linear regression.The results showed that the model explained 36.5%of anxiety variation.Resilience was a negative predictor of anxiety symptoms,and the standard regression coefficient was-0.189(P=0.002);symptom rumination was a positive predictor of anxiety symptoms,and the standard regression coefficient was 0.472(P<0.001).The depression score of HIV/AIDS patients was taken as the dependent variable.In the first layer,ethnicity,education level and infection route were included as the control variables.The second step is to include resilience and rumination.Model 1 explained 7.8%of the variance of depression,and model 2 explained 42.0%of the variance of depression.Resilience was a negative predictor of depressive symptoms,and the standard regression coefficient was-0.38(P<0.001);symptom rumination was a positive predictor of depressive symptoms,and the standard regression coefficient was 0.354(P<0.001).Conclusion:1.HIV/AIDS prisoners presented relatively higer level of anxiety and depressive symptoms.2.There was an significant negative correlation between anxiety symptoms and resilience,between depression symptoms and resilience.Anxiety symptoms was positively correlated with rumination scores and all dimensions,while depressive symptoms was positively correlated with compulsive thinking,symptom rumination and rumination scores,but not with introspection.3.Resilience can negatively predict anxiety and depressive symptoms.Symptoms rumination can positively predict anxiety and depressive symptoms.4.This study can help prison personnel get to know the mental status of HIV/AIDS prisoners timley,provide them with a new thought of psychological intervention to improve their anxiety and depressive symptoms,and improve prisoners' quality of life and treatment compliance to accept education transformation and finish their sentence better.
