Application Research Of Ze Lan Mixture Combined With Electroacupuncture In The ERAS After Elderly Hip Arthroplasty

Objectives:Under the ERAS philosophy,To study the clinical efficacy of Ze lan mixture combined with electroacupuncture therapy on the basis of conventional treatment in the analgesia after hip replacement in the elderly;to explore the application value of Ze lan mixture combined with electroacupuncture therapy in the analgesia after hip replacement in the elderly;to provide new ideas and methods for the multimodal combined analgesia after hip replacement in the elderly.To enrich and explore the new clinical application of traditional Chinese medicine in the field of enhanced recovery.Methods:From January 2018 to January 2019,among the elderly patients who were admitted in our hospital with hip fracture and were operated with unilateral hip replacement operation,after screening according to inclusion criteria and exclusion criteria,72 patients who met the criteria were enrolled in the study.During the study,after the screening of withdrawal criteria,62 cases finally entered the clinical statistics.The treatment group had 30 cases and the control group had 32 cases.The control group had "routine treatment" and the treatment group had "routine treatment + Ze lan mixture combined with electroacupuncture therapy".Visual Analogue Scale and modified Barthel index were observed on the 2nd,6th,10th and 14th day after operation,the addition of analgesics and the average number of days of hospitalization after operation.Statistical analysis of all data was carried out by SPSS23.0 software,and objective evaluation of the efficacy was made.Results:1.before treatment,base data of the two groups such as gender,age,fracture type,operative side,surgical methods,operation time,blood loss,preoperative VAS score had no significant difference,it has comparability.2.Visual Analogue Scale:Follow-up data on the 2nd,6th,10th and 14th day after operation showed that both the treatment group and the control group had a sustained effect on the improvement of pain within two weeks after hip replacement.At the 2th day after operation,there were no significant changes(P=0.884>0.05).At the 6th(P=0.000),10th(P=0.000)and 14th(P=0.001)day after operation,the treatment group had a better effect on the improvement of pain(P<0.05).3.modified Barthel index:both the treatment group and the control group could improve within two weeks after hip replacement;at the 2th day after operation,there were no significant changes(P=0.500>0.05).On the sixth day after operation,the improvement of self-care ability of the patients in the treatment group was significantly better than that in the control group(P=0.007<0.05);and on the 10th(P=0.159)and 14th(P=0.088)day after operation,it was found that although the scores in the treatment group were higher than those in the control group,there was no statistical difference(P>0.05).)4.There was no significant difference in the number of additional analgesics between the two groups during the use of analgesic pump within two days after operation(P=0.443>0.05),but in the number of additional analgesics relying on coxib after operation,the number of additional analgesics in the control group was significantly higher than that in the treatment group,with statistical analysis showing a difference(P=0.000<0.05).5.On the days of hospitalization:the number of patients in the treatment group was(19.54±3.61)days but in the control group was(22.75±4.02)days,and there was significant difference between the two groups(P=0.003<0.05).6.There were no significant changes in the laboratory parameters of the two groups before and after treatment.Conclusions:(1)The combination of Ze lan Mixture and electroacupuncture has definite effect on the improvement of pain in two weeks after hip arthroplasty,and is superior to conventional therapy alone.(2)It can effectively reduce the intake of analgesics so that increase patient satisfaction.(3)In the short term,the improvement of self-care ability of patients after hip arthroplasty is more advantageous than that of conventional therapy alone.However,the advantages will gradually diminish with the increase of time.(4)It can accelerate the rehabilitation of patients and shorten the hospitalization days after hip replacement.,make patients discharged as soon as possible.(5)It is simple,safe and reliable,which is a multimodal analgesia method worthy of clinical promotion.
