Abstract/Summary:
Objective:To explore the value of 320-row CT perfusion imaging in the differential diagnosis of benign and malignant lymph nodes in head and neck,and provide an important basis for clinical diagnosis,treatment and prognosis.Methods:Forty five patients with head and neck lymphadenopathy were collected from October 2017 to December 2018 in our hospital.According to the pathological results,45 cases of lymph nodes were divided into good and malignant groups.The ipsilateral normal sternocleidomastoid muscle was used as the control group.Using the perfusion technique of Toshiba 320-row CT machine,the relationship between BF,BV and PBV values in benign,malignant lymph nodes and normal muscles was analyzed.The morphological characteristics of TDC curves between benign and malignant lymph nodes and the differences between TTP and REV values were compared.Morphological characteristics of benign and malignant lymph nodes were analyzed on a two-dimensional image.Data were statistically analyzed using SPSS 22.0 statistical software and P<0.05 was considered statistically significant.Results:1.The BF,BV and PBV values of benign and malignant lymph nodes in the head and neck were significantly different from those in normal muscle groups(P<0.05).The BF,BV and PBV values of malignant lymph nodes were significantly higher than those of benign lesions and normal muscle groups,and benign.The BF,BV,and PBV values of the lesions were also higher than those of the normal muscle group.The BF value of lymphoma was higher than that of metastatic lymph nodes,and the difference was statistically significant(P<0.05).There were no significant differences in BV and PBV between lymphoma and metastatic lymph nodes(p>0.05).There were no significant differences in BF,BV and PBV between benign groups(p>0.05).2.The TDC of benign lymph nodes was mainly "low level"(55.56%);the TDC of lymphoma was mainly "low level"(66.67%);the TDC of metastatic lymph node was"speed" The rate of slowing down is mainly(46.67%).3.The TTP value of the malignant group was lower than that of the benign group,and there was a significant difference(P<0.05).The TTP value of lymph node tuberculosis was higher than that of acute lymphadenitis,and the difference was statistically significant(P<0.05).There was no significant difference in REV values between acute lymphadenitis and lymph node tuberculosis(p>0.05).There was no significant difference in REV and TTP between the malignant groups(p>0.05).4.The L/S value of benign lymph nodes is more than 2,the margin is more clear,the lymph node is present,necrosis is rare;the L/S value of malignant lymph nodes is more than 2,the edge is more blurred,the lymph node disappears,and necrosis is more common.Conclusion:1.The perfusion parameters of 320-row CT perfusion imaging were different in benign and malignant lymph nodes of the head and neck.2.The TTP value in the TDC has diagnostic significance for acute lymphadenitis.3.320-row CT perfusion imaging combined with CT morphological features of lymph nodes is important to improve the differential diagnosis of benign and malignant lymph nodes in the head and neck.
