Objective:To compare the effects of CPAP combined with different doses of metformin on body weight,blood glucose,blood lipids,pulmonary ventilation function,insulin resistance index and serum lactic acid of obese type 2 diabetic patients with obstructive sleep apnea-hypopnea syndrome(OSAHS).Methods:60 cases of obese type 2 diabetes mellitus patients with obstructive sleep apnea hypopnea syndrome from June 2017 to January 2019 were randomly divided into two groups:a group of CPAP combined with standard dose metformin(standard dose group,30 cases),a group of CPAP combined with low-dose metformin(low-dose group,30 cases).Tne FPG,PPG,HbAlc,blood lipid,IL-6,TNF-a,leptin,adiponectin,HOMA-IR,BMI,AHI,LSp02,clinical efficacy,adverse reactions of the two groups were statistically analyzed.Results:The FPG,PPG levels of the standard dose group were significantly lower than the low dose group(P<0.05),the TG and LDL-C levels were significantly lower than the low dose group(P<0.05),the HDL-C level was significantly higher than the low dose group(P<0.05),the AHI and HOMA-IR was significantly lower than the low dose group(P<0.05),the LSp02 was significantly higher than the low dose group(P<0.05),the IL-6,TNF-a and leptin levels were significantly lower than the low dose group(P<0.05),the adiponectin level was significantly higher than the low dose group(P<0.05),the total effective rate 96.7%(29/30)was significantly higher than the low dose group 66.7%(20/30)(P<0.05).The incidence of adverse reactions 3.3%(1/30)was significantly lower than the low dose group 20.0%(6/30)(P<0.05).Conclusion:CPAP+standard dose metformin is superior to CPAP+low dose metformin in reducing FPG,PPG,HOMA-IR,TG,LDL-C,AHI,IL-6,TNF--a and leptin,increasing HDL-C,LSpO2 and adiponectin,improving dizziness and drowsiness,and short-term noninvasive positive pressure ventilation does not aggravate hypoxia and increase lactic acid side effects action takes place.
