Objective: By observing the clinical effects of different prostheses of the mandibular molars with half root resection,increasing the indications of prostheses,in order to provide a reference for clinical application and increasing the preserving rate of the mandibular molar root.Methods: Totally 23 cases are selected from the Department of Prosthodontics of our hospital and from January 2017 to October 2018.They are 11 males and 12females),33-77 years old,average age is 56.19 cases of mandibular first molar and 4 cases of mandibular second molar.An Informed consent was signed by the patients.Thirteen teeth underwent half root resection after periodontal treatment,and the other half root underwent complete root canal treatment was performed on the preserved roots without root canal treatment.After the alveolar bone resorption is stable and the gum is healed,the teeth are repaired by post and core crown?bridge,and overdenture.The follow-up observation was performed for 3 to 24 months,and the clinical effect was assessed by a term of success or failure.Successful Treatment: The function of mastication is good,no food impaction,patient satisfaction;prosthesis is stable,good marginal adaption,good adjacency relation,no tooth mobility and root resorption;no obvious gingival inflammation,no obvious periodontal pocket.X-ray: normal parodontium space space,no shadow of apical zone,and increased the height and density of alveolar bone in the extraction area.Failure of treatment: patient discomfort,weak function of mastication,food impaction,the patient is not satisfied;unstable prosthesis;root resorption,the gum is red or swollen,deep periodontal pocket;wide parodontium space space appeared in dental radiograph,shadow of apical zone,alveolar bone resorption in the extraction area.Results: Follow up 3 to 24 months,22 cases which underwent half root resection?root canal treatment and different fixed prostheses have been successful cured.The patient had no apparent symptoms,good mastication function,good marginal adaption,good retention,no obvious gingival inflammation,no obvious periodontal pockets,no obvious tooth mobility and root resorption,X-ray: normal parodontium space,no shadow of apical zone and the alveolar bone density is increased in the extraction area.Patients are satisfactory for the effect of prostheses treatment.The success rate is95.65%.The remaining 1 teeth were failure,and the bite pain was occurred after 12 months,because of the occlusal trauma.The root loose I °,X-ray was wider periodontal membrane.Gradually recovered after occlusal adjustment.Conclusion: After half-resection of the mandibular molar,the using of post and core?crown and bridge?overdenture,and other fixed prostheses methods can prolong the preserving rate of root,reduce the alveolar bone resorption and improve the chewing ability of the patient.
