Objective: Severe craniocerebral injury is one of the acute craniocerebral diseases with high disability.The general consciousness of patients is lost,and brain tissue structure is extensively damaged,which poses a great threat to the quality of life for patients,it will also be to increase the financial burden on the patient's family.At present,there is no better treatment for patients with severe brain injury,and secondary damage to craniocerebral injury is a serious problem affecting the quality of patient life.But in clinical practice,we found that Chinese medicine preparation Xingnaojing,it has not only certain curative effect in the treatment of severe craniocerebral injury but also its secondary neurological damage.So we consulting the literature,found that Chinese medicine treatment of craniocerebral injury is divided into two aspects: brain refreshing and blood circulation,they all through that improving blood flow and reducing edema to achieve therapeutic effects.The mechanism of reducing edema may be related with aquaporin 4,so we are building the brain injury model,to observe the effects of refreshing method and traditional blood-activating method on reduce brain tissue edema,apoptosis and aquaporin 4(AQP4)expression in rats,and the brain-wakening method was further studied in the treatment of craniocerebral injury.The efficacy and preliminary exploration of its possible mechanism of action.Methods: Adult Sprague-Dawley rats were randomly divided into control(12rats),model(24 rats),refreshing method(24 rats),activating blood circulation method(24 rats).Severe traumatic brain injury model was made in improved Feeney's way.In the refreshing treatment group,we used the intraperitoneal injection of xingnaojing to refresh the brain.And in the traditional blood circulation group,we used thetraditional Chinese medicine to administer the experiment.Brain samples were collected on 24 th hour,48 th hour,72 th hour,and 7th day.In all groups,brain samples were collected for measurement of water content and HE staining observed the changes in brain tissue.TUNEL method was to detect apoptosis cells and immunohistochemical method to detect the expression of AQP4 after injury.Besides RT-PCR validated the AQP4 expression levels in the groups.Results: The edema content of brain tissue,the apoptosis number around the necrotic of focal brain injury and the expression of AQP4 at different time in the model group were higher than those in the control group(P<0.05),the edema content of brain tissue,the expression of AQP4 and the level of apoptosis in the refreshing method group and activating blood circulation method group were lower than those in the model group(P<0.05),and the RT-PCR results can support the AQP4 mRNA results.Conciusion: Xingnaojing can alleviate the degree of cerebral edema after severe head injury and improve the apoptosis of nerve cells after cell swelling.It is superior to the traditional Chinese medicine preparation in the intestine,and the effect of simple enteral administration is not good.The mechanism of action may be related to inhibiting the expression of aquaporin 4 in damaged brain tissue and alleviating brain tissue edema,and further reducing the cell damage.This can provide a theoretical basis for the improvement of Chinese medicine treatment programs.
