Effect Of Short-term Intensive Insulin Therapy On Circadian Rhythm And Clock MRNA Level In Newly Diagnosed Type 2 Diabetic Patients

Objective: Through short-term intensive insulin therapy for newly diagnosed type 2 diabetes mellitus,the changes of blood sugar,sleep quality,day-night-sleep phase and Clock gene level were observed,and the effects of short-term intensive insulin therapy on sleep quality,day-night-sleep phase and Clock gene level were explored,which provided a new theoretical basis for the clinical application of short-term intensive insulin therapy for type 2 diabetes mellitus.Methods: Sixty newly diagnosed type 2 diabetes mellitus patients admitted to the Department of Endocrinology,Second Affiliated Hospital of Xinjiang Medical University from November 2017 to April 2018 were treated with short-term intensive insulin therapy.Fasting blood glucose(FPG),2-hour postprandial blood glucose(2hPG),fasting insulin(FIns),triglyceride(TG)were compared before and after treatment.Changes of total cholesterol(TC),low density lipoprotein(LDL-C),high density lipoprotein(HDL-C),Clock mRNA level,sleep quality(PSQI),and MEQ(day-night-sleep phase).Results:(1)After short-term intensive insulin treatment,the levels of TG,TC,FPG,2hPG and LDL-C decreased compared with those before treatment,with statistical significance(P < 0.05);the levels of FIns,HDL-C increased,with statistical significance(P<0.05);(2)PSQI scores of patients with T2 DM decreased significantly after treatment,with statistical significance(P<0.05).MEQ score was significantly lower than that before treatment(P<0.05);(3)Clock gene level was significantly higher than that before treatment(P<0.05).Conclusion: In this study,short-term intensive insulin therapy for newly diagnosed type 2 diabetes mellitus can reduce blood sugar,blood lipid and other related indicators,relieve hyperglycemic toxic environment,enhance Clock gene expression,improve sleep quality and regulate sleep-circadian rhythm stability.
