Research goal:Two kinds of topical drugs,epidermal growth factor and elephant skin powder,were used to treat diabetic foot ulcers respectively.The clinical effect of external application of skin powder on diabetic foot treatment was observed.Methods:From September 2016 to June 2018,60 patients with type 2 diabetes mellitus and diabetic foot hospitalized in the seven wards of the Second Hospital of Jiangsu Province(General Surgery).According to the random number table method,60 patients were randomly divided into the treatment group(30 cases)and the control group(30 cases).The patients controlled blood sugar,controlled blood pressure,controlled blood lipids and controlled infection according to the condition.Under the premise of basic treatment,the test group was directly applied to the wound surface with the skin powder,the thickness of the drug was 2-3 mm,covered with sterile gauze,and fixed with bandage No.4;the control group was directly applied with the epidermal growth factor freeze-dried powder.The wound surface was administered at a thickness of 2-3 mm,covered with sterile gauze,and fixed with a bandage No.4.Continuous use for 2 weeks,once a day,measured and recorded the treatment of 0 days,7 days of treatment,14 days of ulcer wound area,wound necrotic tissue and pus area,wound red granular granulation area,sore inflammatory infiltration zone Area,wound exudation,wound surface tissue coverage after treatment,pain index,calculated efficacy index[effective index =(pre-treatment score-post-treatment score)/pre-treatment score×100%],establish a database,use SPSS25.0 software analyzes and compares the statistical test data.Results:1.Basic data:After statistical analysis,there was no significant difference in general data(gender,age,blood glucose,BMI,duration of disease,etc.)between the two groups,and the difference was not statistically significant(P>0.05).Sex.2.Comparison of wound reduction rate scores:The difference between the two groups was statistically significant after 7 days(P<0.05)and 14 days(P<0.05),indicating that the treatment group was superior to the control group in reducing the wound area.3.Comparison of epithelial tissue coverage scores:The difference between the two groups was statistically significant after 7 days(P<0.05)and 14 days(P<0.05),indicating that the treatment group promoted epidermal growth more than the control group.4.Comparison of filling rate of granulation tissue:There were significant differences in treatment of 7 days(P<0.05)and 14 days(P<0.01),indicating that the treatment group was significantly better than the control group in the growth of wound granulation tissue.5.Comparison of intra-week infiltration ratio scores:There was no significant difference between the two groups in the treatment of 0 days(P>0.05).In the treatment group for 7 days(P<0.001),the area of peri-inflammatory infiltration in the treatment group was significantly smaller than that in the control group.In the day(P<0.05),the peri-inflammation infiltration area of the treatment group was smaller than that of the control group,indicating that the treatment group was superior to the control group in anti-inflammatory infiltration6.Comparison of necrotic tissue adhesion rate scores:There was no significant difference between the two groups in the treatment of 0 days.The treatment was 7 days and the treatment was 14 days(P<0.05).The difference was statistically significant.Compared with the control group,the treatment was compared.The clearance rate of necrotic tissue was higher in the group.7.Comparison of wound exudation scores:There was no significant difference between the two groups in the treatment of 0 days(P>0.05).There were significant differences between the two groups in 7 days(P=0.01)and 14 days(P<0.01).The difference indicates that the reduction of wound exudation in the treatment group is significantly better than that in the control group.8.Comparison of pain scores:There was no significant difference between the two groups in the treatment of 0 days(P>0.05),and the difference between the two groups was statistically significant after treatment for 7 days(P<0.05).0.05)There was no significant difference between the two groups,indicating that the treatment group performed better in pain relief at 7 days of treatment.9.Clinical total efficacy control:The comparison of P between the two groups was less than 0.05,the difference was statistically significant,indicating that the treatment of diabetic foot with skin powder is better than epidermal growth factor.Safety index evaluation:There were no adverse reactions such as papules,skin burning sensation and tingling in both groups.Conclusion:Comprehensive comparison of various observation indicators found that elephant skin powder and epidermal growth factor have certain curative effects in the treatment of diabetic foot,but there are significant differences.The skin powder promotes granulation growth,promotes epithelial growth,removes necrotic tissue,and accelerates wound healing.It is superior to epidermal growth factor in healing and has good effects in reducing exudation,anti-inflammatory and analgesia.
