The first part:comparative analysis of MRI and DSA in the diagnosis of primary liver cancer after TACEObjective:to evaluate the effect of magnetic resonance imaging(MRI)on primary liver cancer after TACE.Methods:a retrospective analysis was performed on 60 patients with primary liver cancer who underwent TACE operation.The patients underwent routine MRI plain scan and three phase enhanced scanning before and after TACE one week before and one month after TACE.The results were compared with those of DSA.The signal characteristics of the lesions on MRI sequences were observed.Results:87 lesions were detected by MRI in 60 patients.The lesions of complete embolization of lipiodol,incomplete embolism and distant new lesions were 27,35 and 24 cases,respectively,and 85 lesions were detected by DSA,and the lesions were completely embolized by lipiodol.The number of incomplete embolism lesions and distant new lesions were 25,38 and 22 cases respectively.There was no significant difference between MRI and DSA(P>0.05).Conclusion:with DSA as the gold standard,Mr imaging can accurately analyze the signal characteristics of necrotic foci and recurrent or residual lesions after TACE in the tumor area,and can also play a good role in evaluating the early effect of hepatic artery catheterization and embolization for primary liver cancer.The second part Application of apparent diffusion coefficient of magnetic resonance in the evaluation of curative effect of primary liver cancer after TACEObjective:to study the value of apparent diffusion coefficient(TACE)in the treatment of primary liver cancer(HCC)after TACE.Methods:Sixty patients with primary liver cancer treated with TACE in our hospital were collected,with a total of 87 lesions.Routine MRI scan and DWI sequence scanning were performed before and after operation.The signal characteristics of the lesions in DWI sequence after TACE operation were analyzed.The ADC values of tumor focus and newly developed lesion were measured and recorded.The value of ADC value in differentiating residual and necrotic tumor tissues was analyzed by means of operating characteristic curve.Results:Before operation(group I)the mean ADC value was(0.937±0.08)× 10-3mm2/s;complete embolism(group II)and the mean ADC value was(1.481 ±0.15)×10-3mm2/s;Incomplete embolism focus(group III)mean ADC value was(1.226±0.18)×10-3mm2/s;tumor recurrence focus(group IV)mean ADC value was(0.965±0.06)×10-3mm2/s.The artificial characteristic curve was used to analyze the ability of necrotic tissue and survival tissue.When the ADC value of the lesion isl.425 × 10-3mm2/s,the optimal threshold,the sensitivity of the diagnosis of necrotic tissue is80.0%,the specificity is 81.8%,and the maximum area under the ROC curve is0.861,the maximum area.Conclusion:the apparent diffusion coefficient can be measured quantitatively and the active and necrotic tissue components of the tumor can be distinguished effectively,which can be used to evaluate the curative effect of primary liver cancer after TACE.So as to provide a reliable imaging basis for further clinical treatment.
