Clinical Observation On The Treatment Of Lung Cancer With Malignant Pericardial Metastasis By Tube Drainage Combined With Intravasal Chemotherapy And Dexamethasone Intravenous Infusion

Objectives To inquire into the curative effect of the pericardiac drainage through a thin tube and intracavity chemotherapy associated with intravenous dexamethasone in the treatment of malignant pericardial effusion in lung cancer.Methods We choised 41 patients of pericardial effusion in lung cancer which treated in the First Affiliated Hospital of Bengbu Medical College from July 2015 to February 2018.All patients were treated with pericardiocentesis through a disposable central venous catheter under ultrasonic localization.And they were randomly divided into two groups after the drainage amount was less than 50ml in 24 hours:Twenty one cases were allocated in the associated group,with the drug of Cisplatin 30mg,Interleukins-2 one million units,Dexamethasone 5mg in the pericardium,and with intravenous dexamethasone meanwhile;Twenty cases were allocated in the control group with intracavitary chemotherapy alone.Their short-term and adverse effects incidence were observed.Results(1)The two groups had no statistical significance in gender,age,smoking history and pathological type(P>0.05);(2)The pericardiocentesis were successfully implanted into 41 patients without complication of operation,the longest time of catheters were indwelled for 20 days,and there is no red and swollen,effusion,infection,etc.at the punctured location.(3)The efficiency of the combined group was significantly higher than that of the control group,and the differences was statistically significance.(95.45%VS 65%,P<0.05).(4)There was no significant difference in the incidence of adverse reactions between the groups after pericardial injection(?~2=0.75,P>0.05).There was no constrictive pericarditis in two groups.Conclusion Pericardiocentesis through a disposable central venous catheter under ultrasonic localization is a simple and safe operating,it has't tube,effusion,infection and other neopathy as well.Cisplatin intracacitary chemotherapy has been widely used in the treatment of malignant dropsy of serous cavity,The effective rate of combination is higher than that of single drug.It get a better results with Cisplatin30mg,Interleukins-2 one million units,Dexamethasone 5mg in the pericardium,and with or not intravenous dexamethasone,the combination group was better and had less adverse reactions,which is worth to be spread.
