ObjectiveTo investigate the association of obesity index and obesity types with 10-year coronary heart disease risk in Tibetan and Xinjiang population of China.MethodsData from the Study on prevalence of chronic cardiopulmonary disease in Tibet and Xinjiang area,Chinese National Special Fund for Health-Scientific Research in the Public Interest.Using stratified multi-stage random sampling,1 000 participants aged>15 years were enrolled in each district or county,blood pressure,glucose,lipid levels were tested only among the participants 35 years or oler.Diagnosis of the following three obesity indexes:BMI?28Kg/m2,WC>90(male)/85(female)cm and VFI>10.And then the population was allocated into 3 groups by BMI and WC respectively;group A-BMI<24 kg/m2,group B-BMI 24?28kg/m2,group C-BMI>28 kg/m2;group I-WC<85(male)/80(female)cm,group ?-WC 85?90(male)/80?85(female)cm,group ?-WC>90(male)/85(female)cm.Participants was divided five group,non-obesty,general obesity,abdominal obesity,visceral obesity and compound obesity based on the number and the categories of the obesity diagnostic which have been met.The assessment of 10-year CHD risk was the the Chinese Multi-provincial Cohort Study(CMCS)cohort for 10-year coronary heart disease(CHD)risk model,of which variables were age,blood pressure,TC,HDL-C,smoke(yes/no)and diabetes(yes/no).Results8 565 participants aged 35 years or older were recruited from Xinjiang and Tibet regions with international standardized examination,and finally 6 750 participants were eligible for analysis.There were 26.55%(standardized rate 24.60%)participants met the BMI obesity diagnostic criteria,51.04%(standardized rate 47.41%)for WC and 48.92%(standardized rate 45.42%)for VFI.The proportion of non-obesity,general obesity,abdominal obesity,visceral obesity and compound obesity were 37.84%(2 554 cases),0.74%(50 cases),11.87%(801 cases),9.32%(629 cases)and 40.24%(2 716 cases),respectively.Out of all compound obesity,59.90%(1 627/2 716)of participants included all types of the obesity in our study.After adjustment for confounding factors,multivariate analysis found that elevated BMI,WC and VFI were associated with the greatest risk to the 10-year coronary heart disease risk(P for trend<0.0001).Compared with other obesity type,compound obesity was associated with the greatest risk to the 10-year coronary heart disease risk(OR:2.89,95%CI:2.52-3.30).Multivariate logistic analysis also showed that 10-year CHD risk increased with BMI(or WC)in groups with same WC(or BMI)(P<0.01);the highest grade BMI and WC had the highest 10-year CHD risk(Odds ratio=3.43,95%confidence interval:2.96?3.96).ConclusionObesity was popular in Tibet and Xinjiang of China.BMI,as well as WC and VFI were associated with the 10-year CHD risk.Participants with compound obesity(especially both BMI and WC were abnormal)had the greater risk to 10-year coronary heart disease.Using serveral obesity indexes was efficient for screening CHD high-risk group.
