[Objective]To understand the status of behavior formation of residents living in the National Demonstration Areas and evaluate the effect of health behavior formation,so as to provide references for further construction of National Demonstration Areas for Comprehensive Prevention and Control of Non-Communicable Disease.[Methods]This was a cross-sectional survey, from November to December,2016,and a total of 4000 residents, aged 18 or above, from ten National Demonstration Areas, were selected as participants for this study by multi-stage cluster random sampling method.Information on the demographic characteristics, history of chronic diseases, core knowledge of prevention and control of chronic diseases, and related behavior habits, such as smoking, consumption of fresh vegetables and fruits, changes of salt and cooking oil intake, physical activity, sedentary time and so on,self-reported health condition and related factors was collected via questionnaire. In this study,single-factor analysis and multi-factor analysis were used to analyze the data.Unconditional logistic regression and multi-level logistic model were used to identify the related factors for formation of health behavior.[Results]A total of 3891 residents were involved in the final analysis. The residents were well aware of the activities of the National Demonstration Areas and residents'participation was active.About 88.2%of the residents were aware of the National Demonstration Areas,29.0%of residents were aware of core knowledge of prevention and control of chronic diseases and more than 1/3 participated in the National Demonstration Areas activities.Daily consumption of fresh vegetables and fruits accounted for 72.1%and 53.6%of the totally involved residents.Self-control salt intake and cooking oil accounted for 76.1%,66.7%,respectively.Compared with a year ago, reducing consumption of salt and edible oil accounted for 47.0%and 43.2%of the totally involved residents.Estimated the rate of daily smoking in adult men was 31.1%,the rate in women was 1.9%.However,only 16.5%of residents said " Compared with a year ago, physical activity level increased ",The proportion of reaching the physical activity standard was 15.3%,and the proportion of static behavior was 23.9%.Residents' age, sex, social economic status, awareness of National Demonstration Areas activities and core knowledge of prevention and control of chronic diseases, the degree of participation in the Demonstration Area activities was significantly associated with the formation of residents' health behavior. The residents living in the National Demonstration Areas aware of National Demonstration Areas activities were more intent to consume fresh vegetables(OR=1.830,95%CI:1.406?2.390)every day, practice towards salt reduction(OR =1.527,95%CI:1.189?1.960)and oil reduction(OR ?1.744,95%CI:1.329?2.289).The residents who were aware of core knowledge of prevention and control of chronic diseases were more intent to consume fresh vegetables(OR=2.660,95%CI:2.083-3.398)and fruits(OR =1.341,95%CI:1.126?1.598)every day, practice towards salt reduction(OR =1.216,95%CI:1.021?1.448)and oil reduction(OR=1.407,95%CI:1.178?1.680).The residents with higher degree of participation in the National Demonstration Areas activities were more likely to consume fresh fruits every day(high degree:OR?1.872?95%CI:1.533?2.284;medium degree:OR=1.552,95%CI:1.300?1.852),practice towards salt reduction(high degree:OR=2.338,95%CI:1.921?2.844;medium degree:OR=1.577,95%CI:1.324?1.880)and oil reduction(high degree:OR=2.797,95%CI:2.288?3.419;medium degree:OR ?1.619,95%CI:1.349?1.944),and meet physical activity recommendation(high degree:OR =1.516,95%CI:1.152?1.995;medium degree:OR =1.443,95%CI:1.116?1.866).The residents with higher degree of participation the National Demonstration Areas activities had lower proportion of daily smoking(high degree:OR =0.769,95%CI:0.609?0.972;medium degree:OR =0.973,95%CI:0.772?1.226),lower proportion of static behavior(high degree:OR =0.544,95%CI:0.432?0.686;medium degree:OR =0.676,95%CI:0.553?0.825).[Conclusions]The proportion of healthy behavior formation in the Demonstration Areas increased significantly, and the overall behavior index was higher than the national level. The residents who were aware of the National Demonstration Areas activities and core knowledge of prevention and control of chronic diseases were more intent to form health dietary behaviors. The residents with higher degree of participation in the National Demonstration Areas activities towards formation health behaviors. It is still necessary for the residents to enhance the participation in the National Demonstration Areas activities, especially the measures to encourage participation in health exercise.
