Bladder cancer is a common malignant tumor of urinary system,which seriously threatens people's health.For the treatment of bladder cancer,transurethral resection of bladder tumor(TURBT)is the most commonly used treatment method at present,but the recurrence rate of TURBT is high,and it is prone to bleeding,perforation and other complications.Radical cystectomy has a high risk,and has a great impact on the quality of life of patients after operation.At present,transurethral greenlaser photoselective vaporization of bladder tumor(PVBT)is more and more widely used in clinical practice.This article will discuss the prognostic effect of TURBT and PVBT in the treatment of bladder cancer,which is of great significance for clinical selection of appropriate treatment options and judgment of bladder cancer.Objective:By comparing the clinical effects of TURBT and PVBT in the treatment of bladder cancer,the clinical application value and guiding significance of the two surgical methods were discussed,which could provide reference for the choice of treatment methods for bladder cancer.Method:A retrospective analysis was made of 808 cases of bladder cancer hospitalized in our hospital from January 2010 to December 2016 and treated with TURBT or PVBT.According to the number of tumors,pathological grade and depth of invasion,the data were divided into three groups.The recurrence rate of each group was observed and analyzed after two surgical methods,and the effect of two surgical methods on different types of bladder tumors was compared.All data were analyzed by SPSS 23.0 statistical software.Result:By analyzing the data of 808 patients with bladder cancer treated with TURBT or PVBT in our hospital,it was found that the recurrence rate of PVBT was lower than that of TURBT.The difference was statistically significant(P<0.05).Relapse-Free Survival(RFS)in green laser group was higher than that in electrosurgical group,and the difference was statistically significant(P<0.05).The recurrence rate and RFS of patients with single bladder cancer after TURBT and PVBT were not significantly different(P>0.05),but for patients with multiple bladder cancer,the recurrence rate of patients after PVBT was lower than that of TURBT,and the difference was statistically significant(P<0.05),and the RFS of green laser group was higher than that of electrosurgical group(P<0.05).According to the different degree of malignancy,there was n· significant difference in recurrence rate and RFS between TURBT and PVBT for low malignant potential bladder cancer patients(P>0.05),but for low-grade and high-grade bladder cancer patients,the recurrence rate of PVBT patients was lower than that of TURBT patients(P<0.05),and the RFS of green laser group was higher than that of electric resection group and the difference was significant(P<0.05).According to the depth of tumor invasion,the recurrence rate of non muscle-invasive bladder cancer(NMIBC)patients after TURBT and PVBT was lower than that of TURBT,and the difference was statistically significant(P<0.05).RFS in green laser group was higher than that in electrosurgical group,and the difference was statistically significant(P<0.05).But for muscle-invasive bladder cancer,there was no significant difference in recurrence rate and RFS between TURBT and PVBT(P>0.05).Conclusion:Overall,PVBT is better than TURBT in the treatment of bladder cancer.According to the number of tumors in different groups,there is no significant difference between the two surgical methods in the treatment of single bladder cancer,but for multiple bladder cancer patients,transurethral greenlaser vaporization of bladder cancer is better than TURBT.According to the different grouping of malignant degree of bladder cancer,there is no significant difference between the two surgical methods for patients with low malignant potential bladder cancer,but for low-grade and high-grade bladder cancer,the curative effect of PVBT is better than that of TURBT.There was no significant difference in the prognosis of MIBC treated by the two surgical methods according to the different grouping of tumor invasion depth,but for NMIBC patients,PVBT was better than TURBT.
